Second generation endometrial ablation using Radio-Frequency for heavy menstrual bleeding (A french multicenter study)

Objectif: évaluer la faisabilité, l'efficacité et déterminer les facteurs de risque d'échec après ablation de l'endomètre par radio-fréquence NovaSure ® (Hologic) dans le cadre de ménométrorragies fonctionnelles. Matériel et méthodes: Nous avons réalisé une étude rétrospective multicentrique sur 185 patientes ayant subi une thermodestruction de l'endomètre par NovaSure ® de Juin 2005 à Décembre 2009. L1analyse des données a concerné les caractéristiques de la population, l'évolution des ménométrorragies, les antécédents obstétricaux, le traitement médical et l'épaisseur endométriale (Canadian Task Force classification 11-2). Résultats: Sur 185 patientes recrutées, 134 dossiers ont été retenus pour l'étude (72%). La médiane d'âge des patientes était de 46 ans (+-5). Avec un suivi moyen de 21 mois (+-9,6L 118 patientes (87,5%) ont été traités avec succès, dont 81 (60%) étaient en aménorrhée, alors que 17 patientes (12%) ont présenté une récidive des saignements. Les patientes satisfaites et très satisfaites représentaient 91% de la cohorte, et 97% des patientes 1 recommanderaient le traitement à une amie. Trois patientes (1,6%) ont eu une perforation en per opératoire et cinq (2,7%) ont présenté une endométrite en post opératoire. Douze patients (9%) ont eu une hystérectomie à la fin du suivi. Les dysménorrhées et !es douleurs pelviennes ont été également améliorées par la procédure dans 70% et 56% des cas, respectivement. La grande multi parité a été le seul facteur associé à un risque plus élevé d'échec (p = 0,022). Conclusion : A 21 mois de suivi, l'ablation de l'endomètre par radio-fréquence a été efficace pour le traitement desménométrorragies fonctionnelles dans 87,5%, en évitant l'hystérectomie dans 91%. En outre, dysménorrhées et algies pelviennes ont été améliorées dans 70% et 56% des cas, respectivement.
