Introducción: A día de hoy existen controversias acerca de qué técnica ortodóncica (lingual o vestibular) produce menor cantidad de efectos adversos. Se ha realizado una revisión sistemática y meta- análisis para determinar cuál de las dos técnicas ortodóncicas presenta menos efectos adversos así como si existen diferencias entre las dos técnicas a nivel cefalométrico. Métodos: Se ha realizado una búsqueda en la base de datos Pubmed (Medline) hasta Octubre 2014. Se han analizado seis variables: dolor, caries, dificultad para comer, dificultad para hablar, déficit en la higiene oral y cambios en los valores cefalométricos. El meta-análisis cumple con las normas PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analysis. La evaluación de la calidad de los estudios no aleatorizados se ha realizado mediante la escala de Newcastle–Ottawa (NOS) y la escala de Jadad para los estudios aleatorizados. Se ha seguido el Manual Cochrane de revisiones sistemáticas de intervenciones versión 5.1.0. Resultados: Se incluyeron 10 artículos. El meta-análisis muestra resultados estadísticamente significativos para la ortodoncia lingual en las variables dolor en la lengua, mejillas y labios (OR=28.32, p<0.001; OR=0.087, p<0.001; y OR= 0.13, p<0.001, respectivamente). Los resultados también fueron significativos para las variables dificultad para hablar (OR= 9.39, p<0.001) e higiene oral (OR= 3.49, p=0.047). Los resultados no mostraron significación estadística para las variables dificultad para comer (OR=3.74; p=0.079) y caries (OR=1,15, p=0,814 Streptococcus mutans; OR=0,67, p=0,515 Lactobacillus). A nivel cefalométrico, existe una tendencia en la ortodoncia lingual de incrementar el ángulo U1-L1/IIA (p=0.101) y reducir el ángulo SN-U1 (p=0.079) pero los resultados no fueron significativos. Conclusiones: Esta revisión sistemática sugiere que los pacientes portadores de ortodoncia lingual presentan más dolor, dificultad para hablar y dificultad para mantener la higiene oral que con la técnica vestibular. Sin embargo, no hemos encontrado suficiente evidencia de que la ortodoncia lingual genere más problemas para comer y más riesgo de caries. A nivel cefalométrico, la ortodoncia lingual no presenta diferencias con respecto a la técnica vestibular. No obstante, son necesarios más estudios prospectivos con más pacientes y más tiempo de seguimiento para confirmar estos resultados.
