Même si une intervention précoce est cruciale pour les enfants atteints de troubles du spectre de l´autisme (TSA), de nombreuses familles attendent plusieurs mois, ou plusieurs années, avant de recevoir une intervention comportementale intensive (ICI) dès qu'un diagnostic de TSA est établi. Dans ce contexte, il est essentiel de mettre au point des interventions alternatives efficaces permettant aux enfants placés sur des listes d'attente de commencer leur traitement plus tôt, réduisant ainsi l'impact potentiel des TSA sur leur fonctionnement. La présente étude a évalué le modèle précoce de Denver appliqué par les parents (Parent Early Start Denver Model – PESDM) comme une alternative d’intervention précoce de faible intensité pour les enfants en attente de services d’ICI dans un centre de santé et services sociaux publique situé au Québec, Canada. Vingt-deux enfants ayant reçu un diagnostic de TSA ont été assignés au hasard soit au groupe expérimental (recevant une intervention ESDM), soit au groupe témoin (recevant un traitement communautaire comme d'habitude). Les deux interventions ont eu lieu dans des conditions similaires: 12 mois d’intervention auprès des parents avec une heure par semaine de contact avec le thérapeute. Après 6 mois d'intervention, les thérapeutes du groupe ESDM ont augmenté leur utilisation des techniques d'interaction centrées sur l'enfant, mesurées par les scores de fidélité ESDM. Leurs interventions ont atteint un niveau acceptable de fidélité au ESDM (M = 79%), ce qui indique qu’une intervention en ESDM peut être appliquée avec intégrité dans un centre communautaire et avec un contact de faible intensité avec des professionnels. Bien que les enfants du groupe ESDM aient présenté une augmentation importante du QI (10,1 points), la différence entre les résultats du fonctionnement intellectuel des deux groupes n’atteignait pas la signification statistique.
