El deterioro cognitivo representa uno de los desafíos más significativos en el ámbito socio-sanitario, manifestándose a través de la disminución de funciones mentales como el lenguaje, la memoria, la atención, el pensamiento y el cálculo. Para abordar este problema, se han desarrollado herramientas de diagnóstico temprano debido a su alta prevalencia. Con el objetivo de analizar la eficacia del Test de Evaluación Cognitiva de Montreal en comparación con el Mini-Mental Test en la detección temprana del deterioro cognitivo en adultos mayores, se realizó una revisión bibliográfica de tipo narrativa. Se incluyeron artículos completos publicados en los últimos cinco años, sin restricciones de idioma, abarcando diseños metodológicos cuantitativos y evidencia de calidad categorizada en cuartiles del uno al cuatro. La búsqueda se llevó a cabo en bases electrónicas como Pubmed, Science Direct y Scopus, y los datos se organizaron siguiendo los parámetros de la Guía PRISMA. Los resultados indicaron variaciones en la sensibilidad y especificidad entre el MoCa y el MMSE, destacando una mayor sensibilidad del 94% y especificidad del 60% para el primero. Esto sugiere una ventaja en la detección temprana del deterioro cognitivo utilizando el MoCa. En conclusión, el análisis reveló una asociación del declive cognitivo en adultos mayores con la edad y el nivel educativo, observándose un menor desarrollo cognitivo en aquellos con niveles educativos más bajos. Sin embargo, se identificaron limitaciones en la disponibilidad de artículos que cumplieran con los criterios de inclusión, así como una escasez de investigaciones centradas en la comparación entre el MoCA y el MMSE.
