Evaluar la eficacia de la toxina botulínica tipo A en la prevención de cicatrices patológicas, calidad de la cicatriz, seguridad y satisfacción del paciente Material y métodos: Se realizó una revisión sistemática en Pubmed, Scopus, Embase y Web of Science, incluyendo ensayos controlados aleatorizados y estudios de intervención no aleatorizados (Cohortes). Empleamos RevMan Web para elaborar los gráficos de Forest Plot, usando el Riesgo Relativo (RR) para desenlaces dicotómicos y la Diferencia de media (DM) junto con la Desviación Estándar (DS) para los desenlaces continuos. Se emplearon las herramientas ROB- 2 y ROBINS-I para la evaluación de riesgo de sesgo. Las escalas evaluadas fueron la Escala Vancouver (VSS), la Escala Visual Análoga (VAS), el ancho de cicatriz, la Escala de Stony Brook (SBSES), la Escala de Evaluación de Cicatrices del Observador (OSAS), la Escala de Evaluación de Cicatrices del Paciente (PSAS), la satisfacción del paciente y los efectos adversos. Resultados: Los resultados del metanálisis muestran que la intervención evaluada favoreció al grupo experimental en varios desenlaces; en VSS se observó una diferencia de medias (DM) de -1.07 (IC 95%: -1.59 a -0.56; p < 0.0001), en la VAS una DM de 1.18 (IC 95%: 0.94 a 1.42; p < 0.00001), en el ancho de cicatriz una DM de -0.21 (IC 95%: -0.33 a -0.09; p = 0.0008), en SBSES una DM de 1.64 (IC 95%: 0.78 a 2.50; p = 0.0002), y OSAS una DM de -1.28 (IC 95%: -2.02 a -0.53; p = 0.0008). No se encontraron diferencias significativas PSAS (DM = 0.51; IC 95%: - 1.74 a 2.77; p = 0.65), mientras que la satisfacción del paciente favoreció al grupo experimental (RR = 1.83; IC 95%: 1.10 a 3.07; p = 0.02). Por último, los efectos adversos no mostraron diferencias significativas entre los grupos (RR = 1.59; IC 95%: 0.47 a 5.40; p = 0.46). Conclusiones: Este meta-análisis confirma que la toxina botulínica tipo A es eficaz y segura para mejorar la calidad de las cicatrices quirúrgicas y traumáticas, favoreciendo la calidad, estética y generando alta satisfacción en los pacientes, sin efectos adversos graves. Sin embargo, se requieren ensayos clínicos más robustos y a mayor escala para confirmar su eficacia de manera concluyente
