Pendant longtemps, l'évaluation de la valeur d un médicament et de sa place dans la stratégie de prise en charge d'une pathologie en vue d un remboursement était nettement distincte de l'évaluation scientifique et secondaire à cette dernière. Cette deuxième évaluation était couramment évoquée comme étant le 4ème obstacle à la mise sur le marché d un médicament, après les trois évaluations indispensables de la qualité, de l'efficacité et de la sécurité. Cependant les évaluations de valeur relative, qu elles soient d efficacité ou de coût, deviennent de plus en plus intriquées dans le processus d enregistrement et de commercialisation. Après une présentation générale de l Evaluation des Technologies de Santé, nous décrivons son utilisation dans les processus décisionnels des autorités de santé du monde en nous focalisant plus particulièrement sur le Royaume-Uni, la France, l Allemagne, l Australie, le Canada et les Etats-Unis. Puis nous discuterons les limites de l utilisation de l Evaluation des Technologies de Santé et de ses conséquences pour les compagnies pharmaceutiques souhaitant enregistrer et commercialiser leurs produits dans ces pays.
