Au Québec, comme ailleurs, l'hôpital devient un lieu de passage et le recouvrement de la santé se fait de plus en plus à domicile. S'ajoute à cela la préséance de l'approche résultat des soins. Ces éléments contextuels font émerger un questionnement quant à la notion de continuité qui est un élément fondamental de la pratique des soins infirmiers. Plusieurs écrits font part, non seulement des bénéfices mais aussi des obstacles à la continuité des soins. Cependant, les auteurs reconnaissent la complexité de cette notion et le manque de consensus quant à sa définition claire et précise. La présente recherche, exploratoire, inductive et interprétative, s'inspire d'une méthode ethnographique pour rendre compte de la réalité que vivent des infirmières en soins à domicile dans un CLSC-CH-CHSLD en regard de la continuité des soins. Les objectifs de la recherche sont (1) décrire le contexte dans lequel oeuvrent les infirmières des soins à domicile, (2) identifier des éléments qui sous-tendent le processus de la continuité des soins et les conditions requises pour sa réalisation, (3) relever des moyens utilisés par les infirmières pour assurer la continuité des soins dans leur pratique quotidienne."--Résumé abrégé par UMI.
