Background: Rheumatoid Arthritis (RA) is an inflammatory autoimmune disease that affects people worldwide. Emerging studies have proposed a key role of circadian clock system in RA. Lifestyle traits such as dietary intake and cigarette smoking can trigger and/or influence RA progression. This study hypothesizes that lifestyle traits could modulate the circadian clock system leading to an impact on the severity and progression of rheumatoid arthritis. Accordingly, optimizing the circadian rhythms through lifestyle interventions may benefit rheumatoid arthritis patients. Methods: The study was conducted in two parts: a preclinical study using RA-like animal model and a cross-sectional population study using 2017-2018 Canadian Community Health Survey (CCHS) data. The preclinical part involved two interventions i.e., Western Diet (WD) and cigarettes smoking (SMK) and our goal was to compare their effect on RA-like animal model groups (WTR, WTR-WD and WTR-SMK) healthy control groups (WTH, WTH-WD, WTH-SMK). The circadian profile and disease severity were assessed using wheel running activity and clinical scoring/histological evaluation, respectively. Mice were sacrificed at four timepoints (N=3/timepoints) to assess the peripheral clock gene expressions. The population study analyzed the relationships between arthritis and lifestyle traits. We also examined the association between lifestyle traits and arthritis-related outcomes in individuals with arthritis. Results: Wheel running activity and clock genes expression indicated a significant change in groups fed with western diet and in groups exposed to cigarette smoking. The change was more pronounced in RA-like animals (WTR-WD and WTR-SMK). Disease progression evidenced by clinical score did not differ significantly between groups, however, histological evaluation showed a more severe tissue damage in WTR-WD and WTR-SMK groups. The population study analysis indicated a significant association between arthritis and diminished sleep quality, and poorer general and mental health. Moreover, a healthy dietary pattern and non-smoking status were associated with better general health and refreshing sleep in individuals with arthritis. However, not all investigated dimensions of sleep health were positively associated. Conclusions: Overall the research findings highlight the impact of examined lifestyle traits on both central and peripheral circadian clock which can have detrimental effects on health outcomes and disease progression in rheumatoid arthritis. These findings carry significant implications and a better understanding of the links, and the underlying molecular mechanisms could help with the clinical management of the disease and improvement of disease outcomes.
