Ces dernières années, la pratique de la médecine générale a vu bon nombre de mutations que ce soit du fait des habitudes de vie des médecins comme de celles des patients. Par ailleurs, le maintien d’une garde de première ligne et la limitation d’un usage " inapproprié " des services d’urgence constituent des enjeux majeurs pour lesquels une solution rapide devait être trouvée. La littérature tant belge qu’internationale a été revue afin de mieux appréhender les modèles de garde de première ligne en Belgique et à l’étranger ainsi que les questions suscitées par ces évolutions. De nombreux pays présentent une professionnalisation de la garde de première ligne par l’émergence de structures spécifiques, telles que les walk-in clinics, les out-of-hours centers, les deputizing services, etc. L’évolution de la garde de première ligne en Belgique emprunte aux différents modèles implémentés il y a quelques années dans des pays tels que le Canada, le Royaume-Uni ou les Pays-Bas. Le modèle soutenu par l’Assurance Maladie- Invalidité est la délégation aux cercles de médecins généralistes de l’organisation de la garde et la création de postes de garde selon des modes organisationnels variés répondant aux spécificités locales.
