Medulla Spinalis Yaralanması Geçiren ve Şiddetli Nöropatik Ağrısı Olan Bir Vakada Diadinamik Akım Uygulaması. Nöropatik Ağrıda Diadinamik Akım

Abstract
Bu vaka sunumunun amacı, medulla spinalis yaralanması (MSY) geçiren, C5 seviyesinde komplet tetrapleji tanısı alan ve şiddetli nöropatik ağrısı olan ancak medikal tedavi ve Transkutanöz Elektriksel Sinir Stimülasyonu (TENS) uygulamasından yeterli fayda göremeyen bir vakada diadinamik akım uygulamasının etkisini incelemekti. MSY sonrası cerrahi tedavi geçiren ve yoğun bakım ünitesinde kalan hasta yaralanmadan yaklaşık iki ay sonra rehabilitasyon merkezimize başvurdu. Fonksiyonel fizyoterapi değerlendirmeleri yapılan hastanın fizyoterapi ve rehabilitasyon programı planlandı. Fizyoterapi programı devam ederken hastanın yatışı sırasında da var olan ve McGill Ağrı Ölçeği’ne göre toplam 46 puan alan, üst ekstremitelerinde lokalize ağrının, şiddeti artarak önemli bir şikâyet haline geldi. Uzman hekim tarafından nöropatik ağrı tanısı konan hastaya medikal tedavi planlandı ve fizyoterapi programına TENS eklendi. Ancak üç ay süre boyunca hastanın ağrısında herhangi bir azalma olmadığı gözlendi. Bunun üzerine ganglion blokajı sağladığı düşünülen ancak kanıt düzeyi düşük olan diadinamik akım uygulaması yapıldı. Uygulamanın ilk gününden itibaren ağrısının azaldığını belirten hastanın, uygulama öncesi toplam 40 puan olan “McGill Ağrı Ölçeği” sonucu 10 seans sonunda 24 puan olarak bulundu. TENS ile “McGill Ağrı Ölçeği”nde 6 puan azalma elde edilirken, diadinamik akım uygulaması ile 16 puanlık bir azalma elde edildi. Sonuç olarak; şiddetli nöropatik ağrısı olan MSY’li vakada 10 seanslık diadinamik akım uygulamasının ağrıyı azalttığı belirlendi.
