OBJECTIF: Les lombalgies sont très fréquentes dans la population générale. Elles occasionnent des coût de santé important quand elles récidivent. En est-il de même pour les militaires? Est-ce que les facteurs de chronicité sont les mêmes pour une unité combattante en opération extérieure (OPEX) que dans la population générale? METHODES: il s'agit d'une étude de Cohorte prospective, au 152e régiment d'infanterie de Colmar d'avril 2011 à avril 2012. Toutes les personnes acceptant de répondre aux auto-questionnaires ont été incluses. Les lombalgies ont été évaluées par des questions spécifiques, et par des échelles: EVA, FABQ, Québec, HAD, Coping. RESULTATS: 113 sujets ont été inclus. La prévalence de lombalgies au départ est plus importante (22,5%) que dans la population générale. Au retour, 41 militaires (36,98%) étaient lombalgiques. Ils avaient plus d'antécédents de lombalgies (p=0,004), plus de lombalgies au départ (p<0,0001), plus de lombalgies pendant la mission (p<0,0001) et pratiquaient moins de sport durant la mission (p=0,018) que les non lombalgiques. Aucune échelle hormis l'EVA n'atteint le seuil pathologique. Seul le score de Québec augmente significativement mais défini une incapacité légère. DISCUSSION: Cette étude permet de mettre en évidence une prévalence de lombalgies avant l'OPEX plus importante que dans la population générale et encore plus importante après l'OPEX. Les facteurs de risque sont les antécédents de lombalgies et la survenue de lombalgies pendant l'OPEX. Les conditions de vie favorisent les lombalgies mais pas la chronicisation. Ces lombalgies sont moins incapacitantes que celle de la population générale. Ces éléments doivent être pris en compte pour établir une politique de prévention primaire pour éviter les récidives, et secondaire pour lutter contre la chronicisation.
