Dispnea sering menjadi masalah utama penyebab rawat inap dan gejala utama yang sering muncul pada pasien Acute Decompensated Heart Failure (ADHF). Dispnea terjadi akibat berbagai gangguan dalam proses pernapasan yang membuat seseorang merasa sulit atau tidak nyaman saat bernapas. Tujuan penulisan ini adalah untuk memaparkan asuhan keperawatan pada pasien ADHF yang mengalami dispnea dengan penerapan terapi single session of 20-minute mindful breathing untuk mengurangi dispnea pasien di Ruang Rawat Inap Jantung RSUP Dr. M. Djamil Padang. Metode penulisan adalah studi kasus dengan pendekatan proses keperawatan dan penerapan EBNP. Berdasarkan pengkajian masalah keperawatan yang ditemukan pola napas tidak efektif, penurunan curah jantung, ketidakstabilan kadar glukosa darah dan intoleransi aktivitas. Intervensi yang dilakukan yaitu manajemen jalan napas, dukungan ventilasi, perawatan jantung akut, manajemen hiperglikemia dan manajemen energi. Intervensi single session of 20-minute mindful breathing dilakukan dengan 1 kali sesi selama 20 menit dengan mefokuskan pikiran pada pernapasan. Pengukuran dispnea dilakukan dengan menggunakan skala dispnea pada kuesioner Edmonton Symptom Assessment System–Revised (ESAS-r). Hasil intervensi menunjukkan rata-rata penurunan skor ESAS-r yaitu 2.3, rata-rata peningkatan SPO2 menjadi 99%, rata-rata terjadi perbaikan pada tekanan darah dan pernapasan dalam rentang normal. Seluruh masalah keperawatan membaik. Kesimpulannya bahwa terapi single session of 20-minute mindful breathing terbukti efektif dalam menurunkan sesak napas pasien ADHF. Dengan demikian disarankan untuk menggunakan terapi single session of 20-minute mindful breathing sebagai alternatif intervensi untuk menurunkan dispnea pada pasien ADHF.
