Introducción: La terapia de espejo es una técnica fisioterapéutica basada en imágenes visuales que se utiliza como herramienta sensoriomotora innovadora para tratar enfermedades relacionadas con el dolor. Objetivo: Describir la efectividad de la terapia de espejo en diversas poblaciones de pacientes con dolor. Métodos: Se realizó una revisión sistemática que incluyó 26 ensayos clínicos aleatorizados obtenidos de las bases de datos: Web of Science, Scopus, Pubmed y Scielo. Se utilizaron los términos de búsqueda “Mirror Movement Therapy AND Pain”. Los criterios de inclusión abarcaron estudios publicados en inglés, español y portugués con textos accesibles, y excluyeron los estudios duplicados. La calidad metodológica se evaluó con la escala PEDro. Resultados: Los estudios revisados aplicaron la terapia de espejo en distintas afecciones: amputaciones (9 artículos), accidentes cerebrovasculares (7), lesiones del nervio mediano (4), síndrome de dolor regional complejo (3), dolor de cuello (1), capsulitis de hombro (1) y fracturas (1). Las evaluaciones emplearon herramientas como la Escala Visual Analógica y el cuestionario breve de dolor de McGill. Conclusiones: La terapia de espejo es una herramienta eficaz para tratar diversos tipos de dolor, ya que favorece la activación de la corteza somatosensitiva y la corteza motora primarias. Su integración con otras intervenciones fisioterapéuticas potencia los resultados, y aumenta su efectividad. Además, su evolución junto con la realidad aumentada ofrece un enfoque prometedor, pues mejora la representación de la imagen corporal mediante la manipulación de colores y movimientos.
