Urinary Screening for-OH Triazolam by FPIA and EIA with Confirmation by GC/MS

Abstract
Trlazolam Is a very short-actlng trlazolobenzodlazeplne wlth sedatlve-hypnotic propertles. Approxlmately 2 % of an oral dose Is excreted unchanged In the urine. The major urlnary metabollte is ~-hydroxytrlazolam glucuronlde (70 % of the dose). The objective of thls study was to characterlze the reactlvlty of <~-hydroxytrlazolam In the urlne benzodlazeplne assay by fluorescence polarlzatlon Immunoassay (FPIA; Abbott TDx) In comparlson with enzyme Immunoassay (EIA; Syva EMIT d.a.u, benzodlazeplne assay), c~-OH trlazolam at 300 ng/mL gave a response equivalent to the 200-ng/mL nordlazepam Abbott calibrator. In the EMIT assay, c~-OH trlazolam gave a response equivalent o the 300-ng/mL callbrator (Syva) at 100-200 ng/mL. Both Immunoassays gave posltlve results in 9 out of 9 urlne speclmens collected from Indlvlduels recelvlng trlazolam. Conflrmatlon was performed by analyzlng for c~-OH trlazolam after enzymatlc hydrolysis and formatlon of a TMS derlvatlve for GC/MS. All urlne specimens were positive for c~-OH trlazolam. In concluslon, both the FPIA and EIA Immunoassay screenlng assays are acceptable for detectlng the presence of ~- OH trlazolam In the urine of patients recelvlng therapeutlc doses of trlazolam.
