Gestational trophoblastic disease and risk of nontrophoblastic cancer later in life

Abstract
Contexte La maladie trophoblastique gestationnelle présente certains facteurs de risque similaires au ceux du cancer non-trophoblastique. Cependant, il n’y a pas assez d’information sur le risque à long terme de cancer non-trophoblastique chez les femmes ayant eu la maladie trophoblastique gestationnelle. Nous avons évalué l'association entre la maladie trophoblastique gestationnelle et le risque du cancer non-trophoblastique à long terme. Méthodes Nous avons mené une étude de cohorte à partir des données du fichier de Maintenance et exploitation des données pour l’étude de la clientèle hospitalière (MED-ÉCHO) en incluant les données sur 1 380 635 femmes ayant été hospitalisées au Québec pour la maladie trophoblastique gestationnelle ou un accouchement entre avril 1989 et mars 2021. L'exposition est la maladie trophoblastique gestationnelle catégorisée en môle hydatiforme, môle invasive et choriocarcinome gestationnel. L’issue d’intérêt est le cancer non-trophoblastique survenu au cours de 32 ans de suivi. Nous avons utilisé des modèles de régression des risques proportionnels de Cox ajustés afin d’estimer l’association entre la maladie trophoblastique gestationnelle et le cancer non-trophoblastique. Résultats Au total, 86 femmes hospitalisées pour la maladie trophoblastique gestationnelle (n = 2 948) et 52 409 femmes hospitalisées pour des accouchements (n = 1 377 687) ont développées des cancers non-trophoblastiques. Les môles hydatiformes (RR ajusté 0,80; IC à 95% 0,63-1,00) n’étaient pas associés statistiquement au risque du cancer non-trophoblastique. Cependant, il y avait une tendance à des associations positives entre les môles invasives (RR non-ajusté 1,31; IC à 95% 0,59-2,92) ou le choriocarcinome gestationnel (RR non-ajusté 2,06; IC à 95% 0.98-4,33) et le cancer non-trophoblastique. Conclusion Les femmes ayant eu les môles hydatiformes ou invasives ne semblent pas avoir un risque élevé de cancer non-trophoblastique à long terme. Cependant, les femmes ayant eu le choriocarcinome gestationnel pourraient avoir un risque à long terme de développer le cancer non-trophoblastique.
