Abstract
Hintergrund: Die komplexe, durch post-exertionelle Malaise (PEM) gekennzeichnete Erkrankung Myalgische Enzephalomyelitis/ Chronisches Fatigue-Syndrom (ME/CFS) stellt die physiotherapeutische Praxis vor die Frage, ob und wie körperliche Aktivität gesteigert werden kann, ohne PEM auszulösen. Während körperliche Aktivität in vielen anderen Krankheitsbildern eine zentrale Rolle spielt, ist ihre Übertragbarkeit bei ME/CFS wegen PEM fraglich. Ziel: Kartierung des Wissensstands zur physiotherapeutisch begleiteter Aktivitätssteigerung bei gleichzeitiger Minimierung des PEM-Risikos. Methoden: Das Scoping Review wurde nach Vorgaben des Joanna Briggs Institute und PRISMA-ScR-Checkliste, mittels PCC-Schemas erstellt. Die Population sind Erwachsene mit ME/CFS nach Canadian Consensus Criteria, International Consensus Criteria oder den Kriterien des Institute Of Medicine. Konzept sind aktive Therapieinhalte unter Berücksichtigung des PEM-Risikos. Kontext ist klinisch rehabilitativer. Systematische Suche in PubMed, Cochrane CENTRAL und PEDro sowie Handsuche. Die Extraktionsdomänen sind Aktivität, PEM, Funktion/Leistungsfähigkeit/Lebensqualität, Patient-Reported Outcomes (PROs), Patient*innensicherheit und Follow-up. Ergebnisse: 9 Quellen wurden eingeschlossen (3 Primärquellen, 6 Kontextquellen). Die Studien zeigten Verbesserungen in PROs, es fehlten standardisierte PEM-Messung mit verzögertem Erfassungsfenster, alltägliche Aktivitätsmaße und Follow-ups. Kontextquellen empfehlen konsistent Pacing, symptomorientierte, niedrige Dosierung, Monitoring als Steuerelement und das Vermeiden starrer Progressionspläne. Schlussfolgerung: Eine behut-same, symptomgeleitete Aktivitätssteigerung erscheint möglich. Es mangelt zurzeit aber an PEM-Objektivierung und Messungen der alltäglichen Aktivität, um die Sicherheit der Interventionen verlässlich zu beurteilen.
