El paro auricular persistente (PAS) es una bradiarritmia poco común caracterizada por la ausencia de actividad auricular eléctrica y mecánica, asociada a procesos degenerativos del miocardio. Su diagnóstico requiere descartar causas reversibles y se basa en electrocardiografía, ecocardiografía y monitorización Holter. El tratamiento de elección es la implantación de un marcapasos transvenoso, aunque su pronóstico sigue siendo reservado. Se presentan tres casos clínicos de PAS idiopático en perros. Dos perros de raza Labrador Retriever, jóvenes, y una hembra geriátrica West Highland White Terrier fueron referidos por episodios sincopales, intolerancia al ejercicio y ascitis. En todos los casos, el ECG reveló bradicardia con ritmo de escape supraventricular y ausencia de ondas P. La ecocardiografía mostró ausencia de contractilidad auricular y regurgitación mitral. En los dos Labradores se implantó un marcapasos endocárdico con mejoría clínica inicial; sin embargo, ambos desarrollaron fallo cardíaco progresivo y fueron finalmente eutanasiados. La necropsia confirmó degeneración auricular severa, fibrosis y pérdida de miocitos. En el tercer caso, se rechazó la implantación de marcapasos y se instauró tratamiento médico paliativo con control clínico periódico. Estos casos ilustran la variabilidad de presentación del PAS, su diagnóstico electrocardiográfico característico, el beneficio sintomático del marcapasos y su pronóstico reservado. Destaca la necesidad de reconocer esta entidad en razas no predispuestas y el papel del diagnóstico temprano y el manejo individualizado.
