Transplantados cardíacos precisam realizar mudanças no estilo de vida além do tratamento medicamentoso. Ademais, podem desenvolver déficits cognitivos e depressão após o transplante. O objetivo desta dissertação foi examinar a relação entre sintomas depressivos, presença de comprometimento cognitivo leve (CCL) e barreiras percebidas para o tratamento e autocuidado em transplantados cardíacos, e identificar as principais barreiras percebidas. Participaram 74 pacientes transplantados que responderam a questionários sociodemográficos, clínicos, ao Montreal Cognitive Assessment - Basic, Patient Health Questionnaire – 9 e ao Inventário de Barreiras Percebidas para o Autocuidado Pós-transplante. Os dados foram analisados no software SPSS e IRaMuTeQ. Os resultados indicaram uma mediana de duas barreiras, com 14,9% apresentando sintomas depressivos moderados ou graves e 10,9% apresentando CCL. Comparações entre pacientes com e sem CCL não indicaram diferenças significativas, mas mulheres relataram maior percepção de barreiras. Análises de correlação mostraram relações positivas e significativas entre sintomas depressivos, CCL e barreiras percebidas para o tratamento e autocuidado. A análise de regressão revelou que CCL, complicações pós-transplante e sintomas depressivos explicaram 36% da variância na percepção de barreiras. Os resultados qualitativos originaram 4 classes de barreiras percebidas, são elas: Desafios de adaptação ao tratamento e autocuidado; Rotina hospitalar e demandas físicas e emocionais; Impacto dos medicamentos na rotina e qualidade de vida; e Efeitos colaterais e impactos na vida diária. Os resultados deste estudo demonstram a importância de questionar sobre as barreiras percebidas para cada paciente e de avaliar fatores psicológicos e cognitivos para melhorar a adesão ao tratamento nessa população.
