45 Role of baby-led weaning in foreign body aspiration and iron-deficiency anemia in infants: A retrospective study

Abstract
Abstract Background Baby-Led Weaning (BLW) is an alternative approach of introducing complementary foods in infants that is growing in popularity among parents. In contrast to traditional spoon-feeding (TSF), self-feeding of finger foods is encouraged from the start of complimentary feeding. Benefits have been described, but are mostly based on expert opinion. This approach raises concerns in some paediatricians, particularly regarding the potential risks of foreign body aspiration (FBA) and iron-deficiency anemia (IDA). To this date, there are no established guidelines on BLW. Objectives Our primary objective was to assess the prevalence of BLW in cases of FBA and IDA in children aged 6 to 24 months. Our secondary objectives included comparing the consumption of iron-rich foods, milk intake and safety considerations between children feeding with exclusive BLW, TSF or a mixed feeding approach. Design/Methods All patients aged 6 to 24 months, who received a diagnosis of food FBA and/or IDA at our tertiary care centre between January 2015 and October 2020 were included in our retrospective study. Medical history and circumstances surrounding the choking episode were retrieved from the patients’ electronic records. A parental questionnaire was subsequently conducted to document the complementary feeding approach, eating habits and demographic characteristics. Results 33 patients with FBA and 186 patients with IDA were included in our study. Of the 33 FBA, 6 adopted BLW (18.2%), 22 followed TSF (68.7%) and 5 used a mixed feeding approach (15.2%). Among the cases of IDA, 24 followed BLW (12.9%), 127 chose TSF (68.3%) and 35 used a mixed feeding approach (18.8%). The mean age at presentation in months was younger in the BLW group (11.8) compared to the TSF (15.4) and the mixed feeding (15.1) groups. Iron-fortified cereals were offered less frequently in the BLW (62.5%) group compared to the TSF (87.4%) and the mixed feeding (91.4%) groups. Across all groups, a considerable proportion of infants were not offered iron-fortified cereals past 12 months old. Iron-rich foods were not served at every meal for 1 in 3 infants and daily milk intake often exceeded current recommendations. Conclusion Our results suggest that BLW and the mixed feeding approach may be overrepresented in cases of FBA and IDA in infants. However, regardless of the approach of complimentary feeding, many of these cases appear to result from eating patterns that do not meet the existing recommendations. Our study highlights the necessity of improved patient education on safe and adequate nutrition for infants.
