Glucagon-like peptide-1 receptor agonists and co-agonists for weight loss among individuals without diabetes

Abstract
Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are a class of medications that have been used as an antihyperglycemic agent in the management of type 2 diabetes for almost two decades.In recent years, semaglutide, a GLP-1 RA, has garnered significant interest for its potential for inducing weight loss in individuals with diabetes.In the first manuscript of my thesis, I conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) to assess the long-term efficacy and safety of semaglutide in individuals with overweight/obesity without diabetes.I focused on studies that compared onceweekly subcutaneous semaglutide 2.4 mg to placebo, with a follow-up period of at least 68 weeks.The analysis included four RCTs with a total of 3,087 participants, of whom 94.0% had a baseline body mass index of ≥30 kg/m².My findings demonstrated that semaglutide led to significant reductions in both relative (-12.1%) and absolute body weight (-12.3 kg) compared to placebo.Additionally, 33.4% of participants on semaglutide achieved ≥20% weight loss, compared to 2.2% in the placebo group.While the incidence of gastrointestinal adverse events was higher in the semaglutide group, these events were generally mild-to-moderate in severity and did not often lead to treatment discontinuation.These results suggest that semaglutide is both efficacious and safe for long-term weight loss in individuals with overweight/obesity without diabetes.Building on the weight loss effects of semaglutide in individuals without diabetes, I aimed to expand the scope of my research in the second manuscript of my thesis.Specifically, I compared the efficacy and safety of various GLP-1 RAs and emerging dual and triple co-agonists in the treatment of obesity without diabetes via systematic review.I included 26 RCTs with 15,491 participants who were randomized to receive either a GLP-1 RA, a dual/triple co-agonist, or placebo, with treatment durations of at least 16 weeks.This study covered 12 different agents, II including the commercially available drugs liraglutide, semaglutide, and tirzepatide, as well as nine investigational agents.Among these, the greatest weight loss was observed with retatrutide (relative: -22.1%, absolute: -24.4 kg) and tirzepatide (relative: -17.8%, absolute: -20.9 kg), followed by semaglutide (relative: -13.9%, absolute: -13.7 kg).Safety profiles were similar across the agents, with gastrointestinal adverse events being the most common but generally transient and mild-to-moderate in severity.These findings indicate that while GLP-1 RAs are efficacious for weight loss in individuals without diabetes, dual or triple co-agonists may provide superior efficacy by targeting multiple metabolic pathways, representing an area of ongoing research in obesity treatment.Overall, my thesis highlights the expanding role of GLP-1 RAs beyond diabetes management.The observed efficacy, particularly with newer co-agonist therapies, and acceptable safety profile underscores the potential of these treatments in addressing the obesity epidemic.III RÉSUMÉ Les agonistes des récepteurs du peptide-1 de type glucagon (AR du GLP-1) sont une classe de médicaments utilisés comme agents antihyperglycémiques dans la gestion du diabète de type 2 depuis près de deux décennies.Ces dernières années, le sémaglutide, un AR du GLP-1, a suscité un vif intérêt en raison de son potentiel à induire une perte de poids chez les personnes atteintes de diabète.Dans le premier manuscrit de ma thèse, j'ai réalisé une revue systématique et une métaanalyse d'essais contrôlés randomisés (ECR) pour évaluer l'efficacité et la sécurité à long terme du sémaglutide chez les personnes en surpoids/obésité sans diabète.Je me suis concentré sur des études comparant le sémaglutide sous-cutané à 2,4 mg une fois par semaine à un placebo, avec une période de suivi d'au moins 68 semaines.L'analyse a porté sur quatre ECR réunissant au total 3 087 participants, dont 94,0 % avaient un indice de masse corporelle de départ ≥ 30 kg/m².Les résultats ont démontré que le sémaglutide a entraîné des réductions significatives du poids corporel relatif (-12,1 %) et absolu (-12,3 kg) par rapport au placebo.De plus, 33,4 % des participants sous sémaglutide ont atteint une perte de poids ≥ 20 %, contre 2,2 % dans le groupe placebo.Bien la sécurité de divers AR du GLP-1 et des nouveaux co-agonistes doubles et triples dans le traitement de l'obésité sans diabète.J'ai réalisé une revue systématique complète, IV incluant 26 ECR avec 15 491 participants qui ont été randomisés pour recevoir soit un AR du GLP-1, un co-agoniste double/triple, ou un placebo, avec des durées de traitement d'au moins 16 semaines.Cette étude a porté sur 12 agents différents, dont les médicaments disponibles dans le commerce, à savoir le liraglutide, le sémaglutide et le tirzépatide, ainsi que sur neuf agents expérimentaux.Parmi ceux-ci, la perte de poids la plus importante a été observée avec le retatrutide (relatif : -22,1 %, absolu : -24,4 kg) et le tirzépatide (relatif : 17,8 %, absolu : -20,9 kg), suivis du sémaglutide (relatif : -13.9%,
