Introduction : Les services d’urgences (SU) sont durablement engorgés. Une interface ville–hôpital dédiée aux situations semi-urgentes pourrait réduire les passages évitables, accélérer l’accès aux examens et améliorer la continuité des soins. L’objectif principal était d’identifier les motifs pouvant s’inscrire dans une future interface ville–hôpital. Méthodes. Étude descriptive rétrospective monocentrique au SU du CHU de Poitiers. Inclusion de tous les patients âgés de ≥ 15 ans et 3 mois adressés par leur médecin généraliste la première semaine de chaque mois de 2023. Un comité d’adjudication a été créé pour sélectionner les motifs adaptés à une prise en charge ambulatoire. Résultats. Parmi 1 690 patients adressés par leur médecin généraliste, 849 ont été inclus. Les principaux motifs étaient : maintien à domicile difficile/AEG, douleur abdominale, dyspnée, orthopédie/rachis, anomalie du bilan biologique, puis anémie, infectiologie, neurologie, dermatologie et céphalées. Cinq motifs ont été retenus pour l’interface ville hôpital : anémie, TVP, AIT, colique néphrétique et colique hépatique. Des différences ont été mis en évidence entre les systèmes de santé canadien et français. Une semaine type de l’organisation de l’interface ville hôpital au CHU de Poitiers est proposée afin de correspondre au système de soin Français. Conclusion. Les motifs d’adressage par les médecins généralistes permettent de cibler des parcours ambulatoires coordonnés par l’IPA et portés par une équipe interprofessionnelle, susceptibles de désengorger les SU tout en maintenant la sécurité et la continuité des soins. Une évaluation prospective de la mise en œuvre est désormais nécessaire.
