Difficulté de prise en charge des pathologies semi-urgentes en médecine générale : enquête d'opinion sur la création d'une interface ville-hôpital au CHU de Poitiers (86)

Abstract
Introduction : Les médecins généralistes sont régulièrement confrontés à des difficultés dans la prise en charge des pathologies semi-urgentes. Les services d’urgences sont de plus en plus sollicités. Dans ce contexte, la création d’une interface Ville-Hôpital pourrait représenter une alternative organisationnelle pertinente, inspirée du modèle canadien des « Accueils Cliniques ».Méthodes : Une enquête d’opinion a été réalisée entre juillet et décembre 2024 à l’aide d’un questionnaire diffusé auprès des médecins généralistes du département de la Vienne (86). Résultats : Au total, 47 médecins ont répondu. La majorité rapporte des difficultés importantes pour organiser la prise en charge des pathologies semi-urgentes (note médiane 4/5). L’adhésion au projet d’interface Ville-Hôpital est forte avec 42 répondants (89%) attribuant une note ≥ 3/5 à son intérêt. Les indications jugées les plus pertinentes sont la TVP, la suspicion d’appendicite, l’AIT de plus de 48h et la diverticulite. Des réserves concernent toutefois la charge de travail liée à la réévaluation des patients et la nécessité d’un outil simple et opérationnel.Conclusion : Cette enquête suggère un réel intérêt des médecins généralistes pour une interface Ville-Hôpital afin d’améliorer la prise en charge ambulatoire des pathologies semi-urgentes et réduire le recours aux urgences. La faisabilité d’un tel dispositif nécessite une réflexion interprofessionnelle, ainsi que des études plus approfondies.
