Stimulant Use and Depressive Symptoms: Associations with Severity, Symptom Clusters, and Substance Type

ABSTRACT Objectives: This study examines how the number and type of stimulants used associates to depressive symptoms, including severity and specific symptom clusters. Methods: Data from the 2005–2018 National Health and Nutrition Examination Survey (NHANES) were analyzed. Stimulant use was classified by the number of stimulants used: one type or both. Cocaine and methamphetamine use were each compared with nonuse of any types of illegal drugs. Logistic and linear regressions examined associations with depressive symptoms and severity. Results: The study included 28,333 adults, with 2605 (7.93%) reporting depressive symptoms. Compared with nonusers, individuals using one type of stimulant (n=3085) had higher odds of having depressive symptoms [adjusted odds ratio (aOR): 1.36, 95% CI: 1.10, 1.68; d =0.17], greater symptom severity [adjusted mean difference (aCoef): 0.64, 95% CI: 0.42, 0.85; d =0.16], and higher cognitive-affective and somatic symptom scores. Those using both stimulants (n=1629) showed higher odds of depressive symptoms (aOR: 1.84, 95% CI: 1.53, 2.22; d =0.34), greater symptom severity (aCoef: 1.30, 95% CI: 1.01, 1.58; d =0.33), and elevated cognitive-affective and somatic symptom scores. Compared with nonusers, both methamphetamine and cocaine users had significantly increased odds of having depressive symptoms (aOR: 3.26, 95% CI: 1.70, 6.27; d =0.65 for methamphetamine; aOR: 1.25, 95% CI: 1.01, 1.54; d =0.12 for cocaine), with higher severity and cognitive-affective and somatic symptom scores. Conclusion: Our study underscores the association between stimulants type and number and depressive symptoms. Future studies should replicate our results and evaluate the mechanisms underlying this phenomenon. Objectifs : Cette étude examine comment le nombre et le type de stimulants consommés sont associés aux symptômes dépressifs, y compris leur gravité et certains groupes de symptômes spécifiques. Méthodes : Les données de l’enquête nationale sur la santé et la nutrition de 2005-2018 ont été analysées. La consommation de stimulants a été classée en fonction du nombre de stimulants consommés : un seul type ou les deux.
