Implementation of a health-promoting school program in Belo Horizonte: systematization and adaptation of the operational model

Abstract
The Affordable Health Initiative's health-promoting school model (AHI HPS model) incorporates the principles of the Ottawa Charter from the World Health Organization (WHO) and UNESCO's Health-Promoting Schools initiative. Recognizing the significance of this initiative in driving social transformation and reducing inequalities, a team of researchers from the School of Dentistry at the Universidade Federal de Minas Gerais, in collaboration with the Affordable Health Initiative (AHI) from the United Kingdom, is implementing the AHI HPS model in Belo Horizonte. Therefore, this study aimed to systematize, translate, and adapt the operational model to the context of schools in Belo Horizonte as part of the implementation research of AHI HPS model within this municipality. A universalist approach was adopted, including the steps of semantic equivalence, which involves the systematization of the operational model, translation, synthesis, review by the Expert Committee, and approval of the final version by the author of the AHI HPS operational model. Initially, the systematization of the original operational model, available in the English language on the AHI web platform, was carried out. Following that, the translation into Brazilian Portuguese was independently conducted by two bilingual translators. The synthesis of the translation was submitted to a Committee of 23 basic education teachers through a digital form (Google Forms®), divided into three blocks and further subdivided into smaller parts. The operational model was evaluated for language, clarity, and text comprehension until a 75% agreement was achieved. Perceptions and suggestions were recorded in the comments. In the first round, only the part "Run life and health coaching" in Block 1 had agreement less than 75% regarding clarity. The teachers indicated that the term "coaching" referred to a teaching model not adopted in schools. The text was rewritten and organized to facilitate the description of this section of the operational model. Furthermore, the review incorporated textual and orthographic changes, replacements, or clarifications of terms or words that were not well understood, descriptions of concepts, and adjustments to adapt interventions to the local context and in alignment with current policies. In the second round, there was 100% agreement for all criteria, and no new comments were received. Therefore, the revised version of the operational model was sent to the author of AHI HPS model for evaluation and approval. The author suggested some changes to the titles of the sections of the operational model, which were incorporated: "Run life and health coaching" (introduction, e-learning, quiz, debate, group discussion, individual goal) of Blocks 1 and 2 were changed to "Life Skills Literacy and Health Literacy"; "Sports Tournaments" became "Physical Activities Outside of School (Sports)"; "Active Commuting" was changed to "Physical Activities Outside of School (Active Commuting)". In conclusion, the operational model of AHI HPS model, translated into Brazilian Portuguese, has been culturally adapted to the reality of Brazil, encompassing local school policies, and it will be a relevant technical product for the implementation of the intervention in schools in Belo Horizonte.
