Vitamin B12 deficiency and its association with body adiposity, muscle function and food intake in kidney transplant recipients

Após o transplante renal (TxR), o ganho de peso é frequente e as doenças cardiovasculares (DCV) destacam-se como importante causa de mortalidade. Na população em geral, estudos sugerem que a obesidade está associada à deficiência de vitamina B12 (B12), podendo favorecer a hiperhomocisteinemia (HHcy), um fator de risco para DCV. Em receptores de TxR (RTxR), a associação da B12 com a adiposidade corporal não é conhecida. Avaliar a frequência de deficiência de B12 em RTxR e sua associação com ingestão alimentar, adiposidade corporal, homocisteína (Hcy) sérica, resistência à insulina, marcadores inflamatórios, massa e função muscular e déficit cognitivo leve. Estudo transversal com RTxR (18-70 anos) submetidos ao TxR ≥ 6 meses. Critérios de exclusão: diálise, AIDS, câncer, doenças autoimunes e uso de suplementação de B12. A deficiência de B12 foi definida como valores séricos <200pg/mL e HHcy como Hcy >15µmol/L. A antropometria inclui: índice de massa corporal (IMC) e de adiposidade corporal (IAC), perímetro da cintura (PC) e razão cintura estatura (RCE). Absorciometria radiológica de dupla energia (DXA) para determinação do % adiposidade corporal total (%ACT) e no tronco (%ACTronco). Consumo alimentar: 3 recordatórios 24h. As variáveis laboratoriais avaliadas foram: glicose, insulina, Hcy, proteína C-reativa ultra-sensível e adiponectina. A taxa de filtração glomerular foi estimada (TFGe) pela equação CKD-EPI. A massa muscular foi avaliada pelo índice de massa muscular esquelética obtido por DXA. Função muscular: força de preensão manual e velocidade de marcha. Déficit cognitivo leve determinado pela Avaliação cognitiva de Montreal. Foram avaliados 225 RTxR (125 homens), 49(42 56) anos, 103,5(26,5 169,5) meses pós-TxR, TFGe 52,2±1,4ml/min. A deficiência de B12 foi observada em 31 RTxR (14%). O grupo com estado nutricional de B12 adequado (GB12A; n=194) apresentou idade, gênero, tempo TxR, etilismo, tabagismo, atividade física, uso de drogas imunossupressoras, TFGe, marcadores inflamatórios, massa e função muscular e déficit cognitivo leve semelhantes ao do grupo com estado nutricional de B12 deficiente (GB12D). A ingestão de B12 foi significativamente menor no GB12D vs. GB12A. O GB12D vs. GB12A apresentou valores mais elevados (p<0,05) do PC: 96(88-102,5) vs. 90(82-100)cm. Nas mulheres, os parâmetros de adiposidade foram maiores (p<0,05) no GB12D vs. GB12A: IMC: 29,6(26,3 33,2) vs. 25,6(22,7 29,1)kg/m2, IAC: 39,1(35,9 41) vs. 33,6(30,1 36,9), PC 102,8(100 114) vs. 86(80 97,7), RCE 0,63(0,61 0,73) vs. 0,55(0,50 0,61), %ACT 47,9±2,3 vs. 40,9±0,9 e %ACTronco 51,3±2,7 vs. 42,5±1,2. A concentração sérica de B12 se associou inversamente com a Hcy sérica (quando TFGe ≥60ml/min). O presente estudo sugere que em RTxR, a prevalência de deficiência de B12 é elevada, estando associada com menor ingestão alimentar de B12, maior adiposidade corporal nas mulheres e concentração mais elevada de Hcy.
