EFICACIA DA DESMOPRESSINA NA REDUÇAO DE SANGRAMENTO EM PACIENTES DE ALTO RISCO SUBMETIDOS A BIOPSIA RENAL: REVISAO SISTEMATICA E METANALISE

Abstract
Introdução: A biópsia renal é um procedimento diagnóstico essencial na Nefrologia, mas está associada a risco de sangramento, especialmente em pacientes de alto risco. Estratégias para reduzir essa complicação são fundamentais. Este estudo avaliou a eficácia da desmopressina (DDAVP) na redução de sangramentos em pacientes de alto risco submetidos à biópsia renal de rim nativo, por meio de revisão sistemática com meta-análise. Métodos: Foi realizada uma busca abrangente nas bases PubMed-MEDLINE, EMBASE, Cochrane, ClinicalTrials.gov e anais de congressos até março de 2025. Foram incluídos estudos que avaliaram o uso de DDAVP para redução de sangramentos em pacientes de alto risco submetidos à biópsia renal nativa e que disponibilizaram dados sobre eventos hemorrágicos. Ensaios clínicos randomizados e estudos observacionais foram incluídos e analisados conjuntamente e separadamente. Dois autores, de forma independente, extraíram os dados e avaliaram a qualidade metodológica com as ferramentas ROB-2 e Newcastle-Ottawa Scale (NOS). O desfecho primário foi o número total de eventos hemorrágicos. Resultados: Oito estudos preencheram os critérios de inclusão, totalizando 2.167 participantes (1.246 no grupo DDAVP e 1.021 no grupo controle). A análise agrupada indicou uma tendência não significativa à redução de sangramentos com DDAVP (OR 0,57; IC95%: 0,21–1,52), tanto para eventos maiores (OR 0,69; IC95%: 0,32–1,51) quanto menores (OR 0,67; IC95%: 0,33–1,35). A ampla variação nos intervalos de confiança e a heterogeneidade elevada limitaram a precisão das estimativas. Os cinco estudos observacionais apresentaram média elevada na escala NOS (7,8 ± 0,44). Dentre os três ensaios clínicos randomizados incluídos, dois apresentaram baixo risco de vieses e um apresentou algumas preocupações. Discussão e Conclusões: As evidências disponíveis não permitem conclusões definitivas sobre a eficácia da desmopressina na prevenção de sangramentos em pacientes de alto risco submetidos à biópsia renal de rim nativo. Embora um possível benefício não possa ser descartado, os achados são limitados por imprecisão e heterogeneidade, ressaltando a necessidade de ensaios randomizados com maior poder estatístico.
