Abstract
A Doença Arterial Obstrutiva Periférica é consequência do processo aterosclerótico levando ao estreitamento e enrijecimento da luz arterial. A presença de marcadores subclínicos de aterosclerose como o índice tornozelo-braquial abaixo de 0,90 demonstrou sua capacidade preditiva para o diagnóstico de aterosclerose. O estudo visou identificar a presença da doença arterial obstrutiva periférica através do índice tornozelo-braquial e sua associação com os fatores de risco cardiovascular em idosos. O delineamento de estudo foi do tipo caso-controle, onde os casos eram aqueles com índice tornozelo-braquial igual ou abaixo de 0,90 e os controles eram indivíduos com índice acima de 0,90. Foram incluídos 62 casos e 125 controles. Os fatores de risco avaliados foram: hipertensão arterial, diabetes, obesidade, tabagismo, atividade física, presença de angina, história familiar de doença cardiovascular, níveis séricos de colesterol total e frações, triglicerídeos, consumo de álcool. A análise estatística foi realizada por meio da análise univariada. Os seguintes fatores apresentaram associação com a doença obstrutiva periférica: angina (OR 2,7 IC 95% 1,3-5,4), angina por esforço físico (OR 2,4 IC 95% 1,2-4,9), angina por estresse (OR 15,8 IC 1,9-131,4), angina em repouso (OR 9,7 IC 95% 3,0-30,6), eletrocardiograma alterado (OR 2,6 IC 95% 1,5-5,4), inversão de onda T (OR 2,8 IC95% 1,5 – 5,4), sobrecarga ventricular esquerda (OR 2,7 IC 95% 1,3-5,2), glicemia > 126 mg/dl (OR 4,1 IC 95% 1,3 12,7). O sintoma angina, alterações eletrocardiográficas sugestivas de doença coronariana crônica e diabetes encontram-se associados a doença arterial obstrutiva periférica em idosos atendidos no CAIMI em Manaus.
