Apesar da elevada prevalência da osteoartrite (OA), não há tratamentos aprovados capazes de reduzir a sua progressão ou protelar a necessidade de cirurgia de prótese total 1, 2. O ozônio (O3) é uma variedade triatômica de oxigênio aplicada ao organismo humano com propósitos terapêuticos, principalmente em doenças crônicas como a AO 3. Contudo, há somente poucos estudos sobre o uso do ozônio intra-articular no tratamento da OA de joelho e eles são somente relatos de casos 3, 4, 5, 6, 7, 8. Objetivos: Determinar se o tratamento da OA de joelho com ozônio intra-articular é mais efetivo do que o tratamento com placebo em relação à redução da dor e à melhora da função articular e da qualidade de vida. Métodos: 98 pacientes, entre 60 e 85 anos, com OA de joelho segundo os critérios do Colégio Americano de Reumatologia, atendidos no ambulatório da Disciplina de Geriatria e Gerontologia da Escola Paulista de Medicina - Universidade Federal de São Paulo (DIGG-EPM-UNIFESP), no Ambulatório da Disciplina de Reumatologia da Faculdade de Medicina da Universidade de Santo Amaro e na Pró-Vida. Os participantes foram randomizados para receberem O3 (n= 61) ou placebo (n= 35) no joelho mais dolorido e com a pior função, uma vez por semana, durante oito semanas consecutivas. Eles foram entrevistados duas vezes durante o tratamento, com um intervalo de quatro semanas entre as entrevistas, além da terceira, oito semanas após a última punção articular. Os instrumentos utilizados para se avaliarem os resultados foram: 1) Escala Visual Analógica (EVA); 2) Índice Funcional de Lequesne (IFL); 3). Teste “Timed Up and Go” (TUG); 4) Questionário de Qualidade de Vida SF-36 (SF-36); 5) “Western Ontario and McMaster Universities Osteoarthritis Index” (WOMAC) e 6) “Geriatric Pain Measure” (GPM). Resultados: Após 8 semanas de tratamento, o ozônio foi mais eficaz do que o placebo em relação à redução da dor, melhora da função articular e da qualidade de vida: EVA (DM = 3,16; p < 0.001); IFL (DM = 4,39; p < 0,001); TUG = sem diferença estatisticamente significativa; SF-36 (SF = -37,72; p < 0,001; EGS = -2,40; p < 0,001; CF = -27,70; p < 0,001; AS = -13,29; p < 0,001); WOMAC (IF = 0,47; p = 0,111) e GPM (DM = 23,10; p < 0,001). Eventos adversos ocorreram em 3 pacientes (2 no grupo placebo e 1 no grupo ozônio) e incluíram somente acidentes de punção. Conclusões: O estudo confirma a eficácia do ozônio em relação à melhora da dor e melhora da função articular e da qualidade de vida em pacientes com OA de joelho.
