Fatores associados à adesão no tratamento por hemodiálise: um estudo transversal

Abstract
Com os avanços nas terapias renais substitutivas, a expectativa de vida dos indivíduos com doença renal crônica (DRC) em tratamento por hemodiálise (HD) tem aumentado. No entanto, comprometimento da função cognitiva e transtornos de humor do tipodepressão podem ocorrer após períodos mais longos de HD, os quais influenciam consideravelmente a qualidade de vida e a adesão às terapias medicamentosas, dietéticas e ao próprio programa de diálise. A prevalência de comprometimento cognitivo e demência é cerca de duas vezes maior entre pacientes tratados por HD quando comparados a população em geral. O presente estudo teve como objetivo associar fatores sociodemográficos, desempenho cognitivo, transtorno depressivo, ganho de peso interdialítico(GPID) e aderência no tratamento por HD. Tratou-se de um estudo transversal que avaliou pacientes acima de 18 anos de idade tratados por HD há mais de três meses. A avaliação do desempenho cognitivo foi realizada com uso dos instrumentos Mini Mental State Examination (MEEM) e Montreal Cognitive Assessment (MoCA), Hospital Anxiety and Depression Scale (HADS) foi aplicado para o rastreio de depressão e a aderência ao tratamento e avaliado pelo The End-Stage Renal Disease Adherence Questionnaire (ESRDAQ) e pela medida do GPID. A associação entre as variáveis preditoras e de desfecho foi avaliada através de análises de regressão com uso do pacote estatístico SPSS 27. Ao analisar os dados verificou-se que a aderência terapêutica apresentou associação significativa aos escores MEEM (β 7.72 (IC 95% 0,16; 15.27, p=0,04) e a escala de ansiedade do HADS (β - 8.83 (IC 95% -15.29; -2.38, p=0,008). O GPID esteve associado ao MEEM (β -0,08 (IC 95% -013; -0,03, p=0,002), MoCA (β -0,04 (IC 95% -0,09; -.0005, p=0,04) e domínios depressão (β 0,05 (IC 95% 0,0007; 0,11, p=0,04) e ansiedade (β 0,06 (0,01; 0,10, p=0,01) do HADS. Declínio cognitivo, ansiedade e depressão estão associados a menor aderência terapêutica em pacientes tratados por HD de manutenção.
