Evaluation of the nutritional impact of antineoplastic treatment of patients with head and neck cancer in the interest of the dentist: systematic review

Abstract
O câncer de cabeça e pescoço, tem mundialmente um crescente aumento, o seu tratamento ainda hoje traz severas complicações orais e consequente impacto negativo na qualidade de vida e no estado nutricional desses pacientes. A desnutrição pós oncoterapia muitas vezes pode estar relacionada aos sintomas deletérios que surgem junto ao tratamento. O protesista bucomaxilofacial que tem o conhecimento dessa área pode atuar junto com a equipe multidisciplinar a fim de reduzir esses efeitos e contribuir na melhora e reabilitação desses pacientes. Visto isso, essa revisão sistemática tem como objetivo avaliar os sintomas pós-tratamento do câncer de cabeça e pescoço, que possam impactar no estado nutricional e que sejam de interesse ao protesista bucomaxilofacial para a reabilitação desses pacientes. Uma busca sistematizada foi realizada nas bases de dados Pubmed, Embase (via Elsevier), Web of Science, Scopus, Lilacs (via BVS) e Cochrane Library, no período de 2011 a 2022, o que resultou em uma amostra inicial de 1853 artigos. As ferramentas do Instituto Joanna Briggs para estudos transversais e a Newcastle Ottawa Scale, foram utilizadas para avaliação do risco de viés. Treze estudos observacionais foram selecionados para a síntese qualitativa. Desses, 5 estudos apresentaram baixo risco de viés, 7 estudos risco de viés moderado e 1 estudo com alto risco de viés. Os desfechos de deglutição, perda de peso, xerostomia e função mastigatória demostraram certeza de evidência baixa, já o desfecho trismo certeza de evidência muito baixa. Entre os principais sintomas avaliados a deglutição foi apresentada em 11 artigos, enquanto 4 artigos avaliaram o trismo. O conteúdo científico reunido demonstrou grande heterogeneidade entre os estudos, e os sintomas que impactam no estado nutricional estão sempre presentes pós oncoterapia.
