Dynamics of brain neurotrophic factor levels in children with motor development delays in early intervention

Abstract
Annotation. Delayed motor development in early childhood is a common problem that affects a child's motor and cognitive skills, as well as their future quality of life. The aim of the study was to assess the dynamics of brain-derived neurotrophic factor (BDNF) levels in children with motor development delay before the implementation of an individualized comprehensive early intervention program and one month after, with a view to determining the potential role of BDNF as a biomarker of neuroplasticity. The study included 56 children aged 7-8 months with motor development delay. The children were divided into two subgroups: 28 full-term children with motor development delay and 28 premature children with motor development delay, taking into account their corrected age. The control group consisted of 25 healthy children aged 7-8 months without motor disorders. The early intervention program included motor development exercises, sensory integrative therapy, and training for parents to work with their children on a daily basis. The study lasted 1 month. To evaluate the effectiveness of the early intervention program, the dynamics of motor skill development were analyzed using the Alberta Infant Motor Scale (AIMS) and the BDNF level in blood serum. For statistical analysis, quantitative indicators were described using median (Me) and interquartile range (IQR). Comparisons between three independent groups were performed using the nonparametric Kruskal-Wallis test. Comparisons between two dependent groups were performed using the nonparametric Wilcoxon test at p<0.05. At the beginning of the study, the AIMS assessment showed a gradual decrease in motor development from the control group to full-term children with delayed motor development and the lowest scores in premature children, reflecting a progressive increase in the severity of motor deficits. One month after early intervention, both groups showed an increase in median motor skill scores, with a statistically significant difference between the study groups after rehabilitation (p<0.001). At the beginning of the study, BDNF levels differed statistically in all groups. The lowest and consistently low scores in the group of premature infants indicate reduced neuroplasticity in premature infants with motor disorders. Under the influence of early intervention, after 1 month, there were significant changes in BDNF in two groups, which may indicate the potential effectiveness of rehabilitation programs in stimulating neuroplastic processes, and BDNF can be considered a sensitive marker for assessing the dynamics of motor function recovery.
