Background: Osteoarthritis (OA) is a major cause of pain and decreased joint function, significantly impacting patient quality of life. Physical activity is recommended as an important part of the management of knee OA because it can help reduce pain and improve physical function. This study aims to examine the relationship between physical activity and the clinical status of knee OA patients as an efforts to improve their quality of life. Methods: This study used a cross-sectional design. A total of 50 respondents who met the inclusion and exclusion criteria were enrolled. Data were collected through interviews using two instruments: the International Physical Activity Questionnaire (IPAQ) to measure physical activity levels, and the Western Ontario and McMaster Universities Arthritis Index (WOMAC) to assess the clinical status of OA patients. Results: Univariate analysis results showed that the average physical activity of respondents was 3639.1 MET-minutes/week, while the average clinical status was 56.20. Bivariate analysis using the Pearson statistical test found a significant relationship between physical activity and clinical status (<0.001) with a moderate correlation strength and a negative direction (r=-0.502). This finding indicates that regular physical activity of light to moderate intensity can improve clinical status. Conclusions: There is a significant correlation between physical activity and the clinical status of knee osteoarthritis patients at Adventist Hospital Bandar Lampung. Keywords: clinical status, knee osteoarthritis, physical activity Latar Belakang: Osteoarthritis (OA) merupakan salah satu penyebab utama nyeri dan penurunan fungsi sendi, yang berdampak signifikan terhadap kualitas hidup patient. Aktivitas fisik direkomendasikan sebagai bagian penting dalam penatalaksanaan OA lutut karena dapat membantu mengurangi nyeri dan meningkatkan fungsi fisik. Penelitian ini bertujuan untuk melihat hubungan antara aktivitas fisik dengan status klinis pasien OA lutut guna mendukung upaya peningkatan kualitas hidup pasien. Metode Penelitian: Penelitian ini menggunakan desain penelitian cross sectional. Sebanyak 50 responden yang memenuhi kriteria inklusi dan eksklusi diikutsertakan dalam penelitian ini. Data dikumpulkan melalui wawancara menggunakan 2 instrumen, yaitu International Physical Activity Questionnaire (IPAQ) untuk mengukur tingkat aktivitas fisik dan Western Ontario and McMaster Universities Arthritis Index (WOMAC) untuk menilai status klinis pasien OA. Hasil: Hasil analisis univariat menunjukkan bahwa rerata aktivitas fisik responden sebesar 3639,1 MET-menit/minggu, sedangkan rerata status klinis diperoleh adalah 56,20. Pada analisis bivariat menggunakan uji statistika Pearson, ditemukan adanya hubungan yang signifikan antara aktivitas fisik dengan status klinis (<0,001) dengan kekuatan korelasi sedang dan arah negatif (r=-0,502). Temuan ini menunjukkan bahwa aktivitas fisik rutin dengan intensitas ringan hingga sedang dapat memperbaiki status klinis. Kesimpulan: Terdapat korelasi bermakna antara aktivitas fisik dengan status klinis pasien osteoarthritis lutut di RS Advent Bandar Lampung.
