Abstract

Objetivo: Avaliar a associação entre polifarmácia e delirium em pessoas idosas hospitalizadas. Métodos: Esta revisão sistemática foi conduzida seguindo as diretrizes PRISMA e registrada no PROSPERO. A busca foi realizada nas bases de dados MEDLINE, Cochrane Library, Scielo, LILACS, BDENF, IBECS, Index PSI e Secretaria de Saúde do Estado de São Paulo, utilizando descritores MeSH e termos relacionados para identificar estudos observacionais de coorte e caso-controle que investigavam a associação entre polifarmácia e delirium em idosos hospitalizados. A qualidade dos estudos foi avaliada pela escala Newcastle-Ottawa. Para a síntese quantitativa, foi realizada uma meta-análise utilizando gráficos de floresta (forest plot) e de funil (funnel plot), permitindo avaliar tanto a magnitude dos efeitos quanto a presença de viés de publicação respectivamente. Resultados: A prevalência de polifarmácia variou entre 25,3% e 79,8%, enquanto a prevalência de delirium variou entre 14,3% e 51,3%. A definição predominante de polifarmácia foi o uso de cinco ou mais medicamentos e o método mais utilizado para diagnóstico de delirium foi o Confusinal Method Assessement. A estimativa combinada revelou um Odds Ratio de 1,31 (IC: 0,82 – 2,09), com valor de teste Q de Cochran (25,45 e p=0,0001) e valor de I²de 80,4%. O gráfico de funil, apresentou distribuição razoavelmente simétrica das estimativas de efeito. Conclusão: A síntese descritiva e a meta-análise revelaram, que embora haja uma associação entre polifarmácia e delirium em idosos internados, a heterogeneidade entre os estudos limita a força dessa associação, assim como a generalização da mesma.
