Ce chapitre compare la prise en compte de six enjeux en soins primaires (politiques, environnementaux, sociaux, santé mentale, intégration des soins et résilience) en Belgique, France, Québec et Suisse. Trente-cinq indicateurs ont été développés et renseignés par des experts pour analyser ces aspects. Les résultats montrent des différences notables : le Québec et la Belgique sont plus avancés dans l’interprofessionnalité et l’intégration des soins, tandis que la France et la Suisse restent plus médico-centrées. Les enjeux environnementaux et la résilience sont globalement peu pris en compte. L’intégration entre soins primaires et santé publique est limitée, bien que jugée essentielle pour la planification sanitaire et la résilience. Par ailleurs, la gouvernance influence ces différences : le Québec, avec une organisation centralisée et régulée, favorise davantage l’innovation en soins primaires que la Suisse, dont le système libéral entrave la coordination interprofessionnelle. L’étude souligne l’importance de politiques adaptées pour renforcer l’accessibilité et la qualité des soins primaires. Elle propose d’approfondir ces analyses et d’inspirer des réformes en s’appuyant sur les meilleures pratiques internationales, afin d’améliorer la résilience et l’efficacité des systèmes de santé face aux défis actuels et futurs.
