An 11 year old spayed female Labrador Retriever presented to Cornell University Ophthalmology Service for emergency evaluation of a four day history of glaucoma affecting her left eye. The patient was initially evaluated at her primary care veterinarian for squinting and increased ocular discharge from her left eye. She was diagnosed with glaucoma OS and medically managed with latanoprost and dorzolamide-timolol. Since she did not respond appropriately to treatment, the patient was referred to Cornell for further evaluation and treatment. On presentation, the patient was bright, alert and responsive. On ophthalmic exam of the left eye, notable findings included buphthalmos, corneal edema and scleral injection. Pupillary light reflex, menace response and dazzle reflex were all absent OS, demonstrating that the patient was blind in that eye. The intraocular pressure OS was elevated at 37 mmHg. Ophthalmic exam of the right eye revealed no significant findings (intraocular pressure OD was 12 mm Hg). Ocular ultrasound showed a hyperechoic mass within the globe. Chest radiographs were negative for metastatic disease. The following morning, the patient underwent an enucleation of her left eye. Afterward, she was continued on dorzolamide-timolol for her right eye. The left eye was submitted to the Comparative Ocular Pathology Laboratory of Wisconsin (COPLOW). Histopathology findings were consistent with uveal malignant melanoma and acute secondary glaucoma. At a recheck exam 4 weeks later, the patient was found to be doing well at home. Intraocular pressure in the right eye was normal and she was discontinued off eye medications.
