Benefit of therapeutic exercise compared to hyaluronic acid in decreasing pain and increasing range of joint mobility in patients with knee osteoarthritis treated in a health care facility rehabilitation setting: a Scoping Review

Abstract
Introducción: La osteoartritis es un problema de salud pública que ha afectado a 528 millones de personas en el mundo en el 2019. La evidencia científica muestra que el ácido hialurónico (AH) se administra oralmente o por inyección, demostrando efectividad en reducir el dolor. La fisioterapia, mediante ejercicios diversos, es efectiva para mejorar la funcionalidad y calidad de vida de las personas. Objetivo: Sintetizar la evidencia científica sobre los beneficios del ejercicio terapéutico comparado con el ácido hialurónico en la disminución del dolor y aumento del rango de movilidad articular en pacientes con osteoartritis de rodilla. Material y Métodos: El diseño de este estudio es una revisión exploratoria o de alcance (Scoping Review). Se realizó la búsqueda e identificación de los artículos relevantes en las siguientes bases de datos desde julio 2004 hasta julio del 2024: EMBASE, PubMed, Clinical Trials y PEDro. Se tomaron en cuenta estudios de ensayos clínicos aleatorios, que incluyeron pacientes con diagnóstico de osteoartritis de rodilla grado I-III según Kellgren-Lawrence y criterios diagnósticos radiológicos según la American College of Rheumatology, que hayan recibido tratamiento fisioterapéutico y de ácido hialurónico. Se incluyeron estudios que miden los resultados a través de la escala visual analógica (EVA), Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) y Knee injury and Osteoarthritis Outcome Score (KOOS); el rango de movimiento articular (ROM) se evaluó con el goniómetro universal. Resultados: Se incluyeron 3 ECA con un total de 281 pacientes; se evidenció el beneficio del ejercicio terapéutico y del AH en la disminución del dolor y aumento del rango de movilidad articular mostrando resultados beneficiosos. Conclusión: Ambos tratamientos reducen el dolor en la osteoartritis de rodilla; el ejercicio brinda beneficios sostenidos y el AH un alivio rápido, con resultados variables en ROM.
