Abstract
Bu çalışmada primer dismenoresi (PD) olan ve olmayan kadınlarda omurga postürü ve lumbopelvik yapının karşılaştırılması amaçlandı. Çalışmaya 77 olgu (PD grubu, n:38, yaş: 21 (18–28), vücut kütle indeksi (VKİ): 20.70 (17.31–28.73)) ; asemptomatik grubu, n:39, yaş: 20 (19–23), VKİ: 20.83 (17.31–27.10) ) dâhil edildi. Fiziksel ve demografik özellikler kaydedildi ve âdet siklusları sorgulandı. Menstrual ağrı şiddeti Görsel Analog Skala (GAS) ile omurga postürleri Valedo®Shape cihazı ve New York Postür Değerlendirme Testi (NYPDT) ile değerlendirildi. Lumbopelvik yapının stabilitesi, mobilitesi ve kaslarının morfolojisi sırasıyla Mcgill gövde kas endurans testleri, Valedo®Shape cihazı ve ultrasonografik (US) görüntüleme ile değerlendirildi. Grupların fiziksel ve demografik özellikleri benzerdi (p>0.05). PD grubunda asemptomatik gruba göre frontal düzlemde lumbal açının daha yüksek olduğu (p: 0.05) ve NYPDT puanlarının daha düşük olduğu bulundu (p<0.001). Gövde kas fleksör, ekstansör, sağ ve sol lateral fleksör kas endurans test skorlarının PD grubunda asemptomatik gruba göre daha düşük olduğu belirlendi (p<0.001). PD grubunun asemptomatik gruba göre sagital sakrum-kalça mobilitesinin (p<0.001) ve frontal lumbal mobilitesinin (p: 0.002) daha az olduğu saptandı. PD grubunun asemptomatik gruba göre transversus abdominus, eksternal ve internal oblik, lumbal multifudus ve diyafram kaslarının kalınlığının daha az olduğu bulundu (p<0.001). Bu çalışmada PD'li kadınların asemptomatik kadınlara göre postürün daha kötü olduğu, frontal düzlemde lumbal açının daha fazla olduğu, lumbopelvik stabilitenin, sakrum-kalça ve lumbal mobilitenin, abdominal, lumbal multifidus ve diyafram kas kalınlıklarının daha az olduğu görüldü. Bu sonuçları PD'li kadınların tedavi yaklaşımlarında göz önünde bulundurmak yararlı olabilir.
