Predictability of interdental papilla reconstruction: a systematic review

Abstract
Objetivos: Determinar la técnica con mayor predictibilidad para la reconstrucción de la papila interdental. Métodos: Esta revisión fue realizada siguiendo la guía PRISMA. Se incluyeron ensayos clínicos aleatorizados (ECAs) y controlados (ECCs) que presentaron tratamientos de reconstrucción de papila interdental entre dientes sin alteración del punto de contacto con un mínimo de 6 meses de seguimiento. Estos estudios fueron buscados, seleccionados y analizados en 4 bases de datos electrónicas (PubMed/MedLine, Cochrane Library, EBSCO, ProQuest) y un registro de ensayos clínicos. Además, en 7 revistas indexadas de alto impacto, lista de referencias de los artículos elegibles y un artículo referido por los asesores. Se agruparon en tratamientos no quirúrgicos(imprimación/aumento) y quirúrgicos, y fueron evaluados mediante los instrumentos de Riesgo de sesgo y escala de Newcastle-Ottawa. Resultados: Sólo 12estudios (9ECAsy 3 ECCs) cumplieron con los criterios para su análisis. Los procedimientos con mejores resultados estadísticamente significativos, para el tratamiento no quirúrgico fueron el ácido hialurónico(AH)presentación gelde2mg/ml AH no reticulado+16mg/ml AH reticulado y AH 20 mg/ml reticulado; y quirúrgico, las técnicas de microtunelización bucal y palatino, Azzi y col., Han y Takei, Arunachalam y col. y Beagle Modificado; y el biomaterial de elección fue el injerto de tejido conectivo. Conclusiones: Las técnicas quirúrgicas conseguirían una mejor reconstrucción parcial de la papila interdental que las no quirúrgicas (imprimación/aumento con AHgel). Asimismo, ambos tratamientos serían predecibles para la reconstrucción papilar.
