The Effectiveness of Different Treatment Methods for Coronal Cracks of Endodontically Untreated Teeth: A Systematic Review

Abstract
Problemos aktualumas ir darbo tikslas. Nepaisant plataus vainikinių dantų įskilimų paplitimo kasdieninėje odontologo praktikoje, mokslinėje literatūroje vis dar nėra pateiktų vieningų rekomendacinių gairių, kokie gydymo metodai yra efektyviausi. Pagrindinis šios sisteminės apžvalgos tyrimo klausimas PICO: „Ar netiesioginės restauracijos, naudojamos vainikiniams įskilimams gydyti, padeda išsaugoti pulpos gyvybingumą lyginant su kitais gydymo metodais ar sekimu be intervencijos?“. Medžiaga ir metodai. Sisteminės literatūros apžvalgos protokolas suformuotas remiantis PRISMA reikalavimais. Mokslinių straipsnių paieška vykdyta elektroninėse duomenų bazėse: „PubMed“ („MEDLINE“), „ScienceDirect“ („ELSEVIER“) ir „Cochrane Library“, bei rankiniu būdu. Kohortinių ir nerandomizuotų klinikinių tyrimų kokybė ir šališkumas įvertintas remiantis Newcastle – Ottawa skale. Rezultatai. Į tyrimą įtraukti 6 straipsniai, atitinkantys apibrėžtus įtraukimo kriterijus bei tyrimo tikslą. Įvertinta atrinktų straipsnių sisteminių klaidų rizika: 5 tyrimai atitinka aukštą kokybę, 1 – vidutinę. Iš viso ištirti 3063 vainikiniai įskilimai endodontiškai negydytuose dantyse, iš kurių 65,75 % (n = 2014) nustatyti apatinio žandikaulio krūminiuose dantyse. Asiptominiams vainikiniams įskilimams taikomas neintervencinis stebėjimas. Tuo tarpu, simptominiams vainikiniams įskilimams taikomas intervencinis gydymas, kuris susideda iš ankstyvojo (pirminio) gydymo, dažniausiai taikant išorinį ar vidinį įtvėrimą, po kurio naudojamas baigiamasis gydymas kauburus gaubiančiomis restauracijomis – vainikėliais ir užklotais. Endodontinio gydymo prireikė tik 3,72 % (n = 114) tiriamųjų. Bendra taikytų gydymo strategijų sėkmė yra 69,31 %. Išvados. Nustačius galutinę pulpos diagnozę simptominiai vainikiniai įskilimai yra sėkmingai gydomi kauburus dengiančios netiesioginėmis restauracijomis – vainikėliais ar užklotais.
