Acute Respiratory Distress Syndrome (ARDS) is a severe pulmonary illness characterized by the disruption of the alveolar-capillary barrier that leads to vascular leakage and pulmonary edema. One important mechanism for vascular leakage is endothelial cell death, while endothelial regeneration helps repair vascular integrity. Multiple types of cell death have been described but their relative contribution to endothelial leakage in ARDS is unknown. The contribution of endothelial regeneration pathways to recovery from ARDS is also poorly characterized. We developed both lethal and sublethal influenza A induced-ARDS models in vivo to compare the frequency and mechanisms of endothelial cell death and regeneration. We observed a significant loss of lung microvascular endothelial cells in the lethal but not the sublethal model and found that microvascular endothelial apoptosis, instead of pyroptosis or necroptosis, may contribute to lethality. Additionally, we found that FoxM1-mediated endothelial regeneration pathways might be impaired in the lethal model. Accordingly, future ARDS therapies should aim to inhibit endothelial apoptosis or augment endothelial regeneration through the FoxM1 pathway.
