Abstract

認知症予防では,積極的な社会参加と予防のための生活習慣を獲得することが重要である。本研究の目的は,サロンに参加する地域在住女性高齢者を対象に,軽度認知障害(Mild Cognitive Impairment:MCI)が疑われる者の特徴と1年後の認知機能低下に影響する因子を明らかにすることである。方法は,初回調査時(初期)のMontreal Cognitive Assessment Japanese version(MoCA-J)を用いて,カットオフ値である26点以上(健常群)と25点以下(MCI群)で,基本属性,精神心理機能,生活機能をMann-Whitneyの検定とFisherの正確確率検定で比較した。さらに,MCI群について,1年後のMoCA-Jの変化量をもとに低下群・維持群・向上群に分け,一元配置分散分析およびFisherの正確確率検定で初期の各変数を比較し,1年間の認知機能低下に関連する因子を検証した。その結果,MCI群の中でも低下群は教育年数が短く,エンパワメントの相互作用による問題の意識化で低下を認めた。また,相談相手が少なく,週毎の外出頻度も低かった。本研究の結果から,認知症予防では,週の外出頻度増加などの行動変容を目的に,集団活動では参加者自身が課題を認識し積極的な行動力を高める支援が重要であると考えられた。
