Changes in Children's Quality of Life and Clinical Indicators of Oral Health Following the Treatment of Molar and Incisor Hypomineralization: A Systematic Literature Review

Abstract
Problemos aktualumas ir darbo tikslas. Krūminių dantų ir kandžių hipomineralizacija reikšmingai veikia su burnos sveikata susijusią gyvenimo kokybę ir turi neigiamos įtakos burnos ertmės sveikatai. Literatūroje dar nėra vieningos nuomonės dėl skirtingų gydymo strategijų poveikio šiems rodikliams. Taigi, šio darbo tikslas yra išanalizuoti ir įvertinti, kaip taikant įvairias gydymo taktikas keičiasi vaikų gyvenimo kokybė bei burnos ertmės sveikatos klinikiniai rodikliai. Medžiaga ir metodai. Publikacijų paieška vykdyta MEDLINE (PubMed), Google Scholar, Wiley Online Library, Web of Science duomenų bazėse. Mokslinės literatūros paieška pradėta 2024 metų rugsėjo 23 dieną, paskutinės paieškos data – 2025 metų sausio 24 diena. Eksperimentinių analitinių tyrimų sisteminių klaidų rizika nustatyta naudojant Cochrane rizikos vertinimo įrankį, o stebėjimo analitiniams tyrimams taikyta Newcastle–Ottawa skalė. Rezultatai. Į mokslinės literatūros apžvalgą įtraukta 11 tyrimų. Septyniuose tyrimuose nustatyti statistiškai reikšmingi su burnos sveikata susijusios gyvenimo kokybės pokyčiai po dantų gydymo. Hipomineralizacijos pažeistų krūminių dantų padengimas silantais, lyginant su kitomis gydymo taktikomis, sąlygojo didžiausius gyvenimo kokybės pokyčius: nuo 14,70 ± 5,90 iki 2,70 ± 3,20 (p < 0,001). Pakitęs priekinių dantų estetinis vaizdas sąlygojo reikšmingai pagerėjusią gyvenimo kokybę: nuo 47,40 ± 9,34 iki 59,80 ± 9,7 (p < 0,001). Penkiuose tyrimuose nustatyti reikšmingi dantų jautrumo pokyčiai: nerūdijančio plieno vainikėliai ir sidabro diamino fluoridas efektyviai slopino padidėjusį hipomineralizacijos pažeistų dantų jautrumą (p < 0,05). Išvados. Krūminių dantų ir kandžių hipomineralizacijos gydymo strategijos statistiškai reikšmingai pagerina vaikų gyvenimo kokybę burnos simptomų, funkcinių apribojimų, emocinės ir socialinės gerovės subkategorijose. Šie metodai sąlygoja reikšmingą skausmo, dantų jautrumo, ėduonies intensyvumo sumažėjimą bei individualios burnos higienos (apnašų indekso) pagerėjimą.
