Darbo tikslas Įvertinti ankstyvuosius ir vėlyvuosius rezultatus, po bestentinio aortos vožtuvo implantavimo naudojant subkoronarinę bei aortos šaknies pakeitimo metodikas Uždaviniai Įvertinti šių operacijų efektyvumą ankstyvajame ir vėlyvajame pooperaciniame laikotarpyje, esant skirtingoms operacinėms technikoms. Įvertinti komplikacijų riziką ankstyvajame ir vėlyvajame pooperaciniame laikotarpyje, taikant skirtingas operacines technikas. Įvertinti echokardiografinius rodiklius, kuomet buvo taikytos skirtingos operacinės technikos, anstyvajame ir vėlyvajame pooperaciniame laikotarpyje. Metodika Retrospektyviai išanalizuoti LSMUL KK Širdies, krūtinės ir kraujagyslių chirurgijos klinikoje 2005 - 2014 m. operuotų 216 pacientų duomenys (vidutinis amžius 73,6 ± 5 m.), kuriems buvo implantuoti bestentiniai aortos vožtuvai dėl vyraujančios aortos stenozės. Pacientai buvo suskirtyti į 2 grupes: I grupės pacientai, tie, kuriems pakeisti aortos vožtuvai naudojant subkoronarinę techniką (n=126 (Toronto SPV)), II grupės pacientai – pakeista visa aortos šaknis, panaudojant biologinę aortos šaknį (n=90 (Medtronic root)). Retrospektyviai buvo analizuojami echoskopiniai tyrimai, atspindintys hemodinaminius pokyčius, kairiojo širdies skilvelio masės regresiją, pacientų išgyvenamumą bei vožtuvo ilgaamžiškumą naudojant skirtingas aortos vožtuvo pakeitimo operacines technikas. Rezultatai Hospitalinis mirštamumas po aortos vožtuvo pakeitimo operacijų buvo 6,9%. Išgyvenamumas praėjus 5 m. po operacijos sudarė 73,4±4%. Laikotarpis be implantuoto vožtuvo infekcinio endokardito ir pakartotinio aortos vožtuvo pakeitimo I ir II grupėse atitinkamai sudarė 92±5,2% ir 99±1,6%, (p<0.02). Taip pat nebuvo registruota jokių struktūrinių vožtuvo pažeidimų ankstyvuoju pooperaciniu laikotarpiu. Vidutinis subkoronarinės procedūros grupės vožtuvinis gradientas po 5 metų siekė 14,7± 8 mmHg, o biologinės aortos šaknies - 12,6 ±5 mmHg. Kairiojo skilvelio hipertrofija ženkliai sumažėjo abiejose ligonių grupėse (p<0.02). Išvados Implantuojami bestentiniai aortos vožtuvai yra saugūs ligoniui remiantis pooperaciniais echoskopiniais tyrimais bei sergamumo ir mirštamumo duomenimis.
