Abstract
Elméleti háttér: A sztóma (mesterségesen kialakított bélnyílás a hasfalon) képzése egy gyakran végzett eljárás a vastag- és vékonybélsebészet során, amely keretében a háttérbetegség kezelése céljából a bél kivezetésre kerül. Mindez hazánkban is alkalmazott eljárás, azonban a sztómával élő személyek műtét utáni alkalmazkodásáról és a megváltozott élethelyzetükben támogatást nyújtó pszichés faktorokról még keveset tudunk. Cél: Jelen kvalitatív, empirikus kutatás célja a sztómával élő személyek sztómaképző műtétet követő, megváltozott élethelyzetükhöz történő hosszú távú alkalmazkodását segítő protektív faktoroknak a feltérképezése. Módszerek: Kutatásunk során a McGill Betegségnarratíva Interjú technikájával vettünk fel interjúkat. Az adatgyűjtésben összesen 11 fő (9 nő, 2 férfi, átlagéletkor 54,7 [SD = 9,62] év) vett részt. Az interjús adatokat a tematikus analízis segítségével elemeztük. Eredmények: Az érintettekkel folytatott interjúk elemzése során számos kódot sikerült elkülöníteni. A kapott kódokat végül öt különböző témába rendeztük, úgymint társas támasz, felsőbbrendű erőbe vetett hit, szakképzett segítség, intraperszonális tényezők, életszemlélet. Az öt témán belül kilenc altémát sikerült azonosítani. Ezekben a témákban jutott kifejezésre a protektív faktorok összessége, amelyről az interjúk résztvevői beszámoltak. Következtetések: A kvalitatív kutatásunk során számos protektív tényezőt sikerült azonosítanunk. A kapott eredmények segíthetik a hazai, sztómával élő populációval kapcsolatos ismereteink bővítését. Az adaptívnak bizonyuló protektív tényezők jobb megismerésével pedig a különböző segítő szakmák képviselői is hatékonyabb támogatást nyújthatnak az érintettek számára. Eredményeink nemzetközi megállapításokkal történő összevetése arra enged következtetni, hogy a feltérképezett tényezők hasonlóak azokhoz a protektív faktorokhoz, amelyeket a nemzetközi szakirodalomban tártak fel. | Theoretical background: Stoma formation (a surgically created opening in the abdominal wall for bowel diversion) is a common procedure in colorectal and small bowel surgery enabling the management of underlying diseases. While this procedure is also commonly used in Hungary, little is known about the post-surgical adaptation of individuals living with a stoma and the factors that provide support in their changed life circumstances. Aim: This qualitative study aims to identify protective factors that facilitate long-term adaptation following stoma surgery. Methods: Using the McGill Illness Narrative Interview, we conducted qualitative interviews with 11 participants (9 women, 2 men, mean age = 54.7 [SD = 9.62] years). The interview data were analyzed using thematic analysis. Results: The analysis of the interviews revealed multiple codes, which were ultimately categorized into five overarching themes: social support, faith in a higher power, professional assistance, intrapersonal factors, and life perspective. Within these five themes, nine subthemes were identified. These encapsulate the protective factors reported by participants. Conclusions: The study identifies crucial protective factors that aid adaptation in individuals with a stoma, expanding knowledge on the Hungarian ostomate population. A deeper understanding of the protective factors that prove to be adaptive may enable professionals from various helping disciplines to provide more effective support for individuals concerned. A comparison of our findings with international results suggests that the factors identified are similar to those protective factors reported in the international literature.
