A Study on Prelabour Rupture of Membrane (PROM) at Rajshahi Medical College Hospital

Abstract
Background: Prelabour rupture of the membrane (PROM) is a common problem faces by the obstetricians. This study was conducted to have a look into the causes and outcome of PROM cases at Rajshahi Medical College Hospital. Methods: This study was conducted from April 2019 to June 2020 at the Department of Obstetrics & Gynaecology of Rajshahi Medical College Hospital, Rajshahi. A total of 76 PROM cases was included; out of them 36 (47%) were preterm PROM and 40 (53%) were term PROM. Their antenatal clinic (ANC) visit records regarding past & present obstetric history were recorded. Urine culture for bacteria was done in all the cases. Outcome of the current pregnancy as mode of delivery, living or dead baby, weight of the baby, APGAR score were recorded. The data was presented as actual number and percentage of total. In addition “Mean ± SD” was used to express the quantitative data. Results: Age range of them was 18 to 36 years and gestational age was 29 to 39 weeks. Among all the cases 42(55%) attended antenatal clinic (ANC) regularly, 30(39.5%) irregularly and 4(5.5%) never attended. 5 cases were with multiple pregnancies, of them 3 in preterm group and 2 in term group. Number of malpresentation was 6. Dead baby was the outcome in 08 (11%) cases, all of them were preterm PROM. Urine culture +ve for bacteria were in 4(5.5%) cases; all of them gave birth to dead babies. Underweight babies were 15 in preterm and 2 in term group; all multiple pregnancies (n=5) were underweight. Delivery by caesarean section was done in 40 cases, normal or assisted vaginal delivery (NVD) was done with live baby in 23 cases and NVD was done with dead baby in 8 cases. Conclusion: Dead baby was the outcome in preterm PROM only where positive urine culture for bacteria came out as a very grave sign.
