Incidence and progression of interstitial lung disease in systemic sclerosis patients without significant interstitial lung disease: associations with immunosuppressive drugs

Contexte: La maladie pulmonaire interstitielle (MPI) est la cause principale de mortalité dans la sclérodermie systémique. Les médicaments immunosuppresseurs sont actuellement utilisés pour le traitement de la MPI, mais sont généralement inefficaces pour renverser le dommage établi. Nos études de cohorte rétrospectives visaient à explorer l'effet des immunosuppresseurs dans les formes légères de la MPI, ainsi qu'à étudier l'association entre l'utilisation des immunosuppresseurs et le risque de MPI incidente, en utilisant les données de patients inscrits dans le registre du Groupe Canadien de Recherche sur la Sclérodermie de 2004 à 2017.Méthode: Tout d'abord, les patients atteints de sclérodermie avec une MPI légère (capacité vitale forcée (CVF) initiale > 85%) (N=116) ont été comparés selon leur exposition au CYC et/ou MMF. La CVF a été évaluée à un an en utilisant un modèle de régression linéaire multivariée. Puis, les patients atteints de sclérodermie sans atteinte pulmonaire à la visite initiale (N=1,131) ont été comparés selon leur exposition au CYC, MMF, azathioprine et/ou méthotrexate. Le délai entre l'exposition et la survenue d'une MPI incidente a été évalué en utilisant un modèle structurel marginal Cox pondéré par la probabilité inverse du traitement.Résultats: Chez les patients atteints de sclérodermie avec une MPI légère, la CVF à un an était plus élevée chez les patients exposés au CYC/MMF comparativement aux patients non-exposés, par une différence de 8.49% (95% CI, 0.01 à 16.98) de la valeur prédite de CVF. La CVF initiale modifiait l'association entre l'exposition au CYC/MMF et la progression de la maladie pulmonaire. Par contre, chez les patients sans atteinte pulmonaire, l'exposition aux médicaments immunosuppresseurs ne réduisait pas significativement le risque de MPI incidente (HR pondéré: 0.78, 95% CI 0.44 à 1.37).Conclusions: Ces résultats supportent l'hypothèse qu'une fenêtre d'opportunité thérapeutique puisse exister dans la MPI associée à la sclérodermie systémique. Le traitement de la MPI à un stade quand les fonctions pulmonaires sont encore normales devrait être considéré comme un moyen de prévenir la maladie pulmonaire progressive, avant que la fonction pulmonaire ne soit trop gravement compromise.
