Abstract
RÉSUMÉLe rôle de l’inductibilité de la fibrillation auriculaire (FA) reste un sujet d’intérêt clinique majeur, en particulier sa valeur après une ablation de tachycardie supraventriculaire (TSV). Des études antérieures ont identifié l’inductibilité de la FA comme un prédicteur de récidive clinique de FA après une ablation du flutter auriculaire (FLA) et comme un potentiel critère de fin d’intervention lors des procédures d’ablation de la FA. Identifier les facteurs prédictifs de l’apparition de la FA après une ablation de TSV est essentiel pour guider les médecins dans le suivi des patients et la gestion des facteurs de risque. En outre, évaluer l’inductibilité de la FA fournit des informations précieuses sur les mécanismes qui initient et entretiennent cette arythmie, contribuant à une meilleure compréhension de son comportement après une ablation de TSV.Cette thèse, basée sur des manuscrits, aborde deux questions importantes.Manuscrit n°1. Déterminer la valeur prédictive de l’inductibilité de la FA après une ablation de TSV. Un protocole standard de stimulation auriculaire par rafale a été utilisé chez des patients subissant une ablation de TSV, et son résultat a été évalué comme prédicteur de récidive clinique de FA. Nous avons constaté que les patients ayant un test d’induction positif après l’ablation de TSV, sans antécédents de FA clinique, présentaient un risque multiplié par 9 de développer une FA future. Ce projet a été présenté au Congrès International ABEC sur les Arythmies, l’Électrophysiologie et la Stimulation Cardiaque Artificielle 2024, où il a remporté le deuxième prix. Le manuscrit a été soumis pour publication dans le Canadian Journal of Cardiology Open.Manuscrit n°2. Comprendre l’utilité de l’inductibilité par stimulation auriculaire par rafale chez des patients subissant une étude électrophysiologique, que ce soit après une ablation de FA, de FLA ou de TSV. Une revue des études disponibles sur PubMed a été réalisée pour répondre à cette question. Le manuscrit sera soumis pour publication dans une revue à comité de lecture
