Eficacia de la vía intraósea vs. vía intravenosa en la parada cardiaca extrahospitalaria: una revisión sistemática y metaanálisis

Abstract
RESUMEN Introducción. La rápida obtención de un acceso vascular es clave en el contexto de la parada cardiorrespiratoria extrahospitalaria. Históricamente, el acceso intravenoso se ha posicionado como la opción más válida, pero las características de los pacientes y el entorno han otorgado un papel relevante y cada vez más utilizado al acceso intraóseo. Objetivos. Evaluar la efectividad de la vía intraósea frente a la vía intravenosa en el contexto de la parada cardiorrespiratoria extrahospitalaria en población adulta. Material y métodos. Se llevó a cabo una revisión sistemática y metaanálisis de estudios observacionales de tipo cohortes retrospectivas. Se extrajo información de: Web of Science TM , Scopus y Cochrane Library. Se obtuvieron datos sobre la recuperación de la circulación espontánea, la supervivencia al alta hospitalaria y la supervivencia al alta hospitalaria con buen resultado neurológico. Se realizó una regresión agrupada de datos anidados bajo el modelo de efectos aleatorios. Se ejecutó tamaño del efecto mediante odds ratio, análisis de heterogeneidad, análisis de subgrupos, análisis de sensibilidad y sesgo de publicación. Resultados. Se incluyeron nueve registros en la síntesis cuantitativa, donde la mayoría de los estudios incluidos alcanzaron puntuaciones altas en la escala Newcastle-Ottawa. Recuperación de la circulación espontánea: OR=0,68 [0,62-0,74] para IC95 % (p<0,00001; I 2 =53 %, p=0,04); supervivencia al alta hospitalaria: OR=0,60 [0,48-0,75] para IC95 % (p<0,00001; I 2 =74 %, p=0,0007); supervivencia al alta hospitalaria con función neurológica favorable: OR=0,53 [0,36-0,76] para IC95 % (p=0,0007; I 2 =81 %, p=0,0004). Conclusiones. La efectividad de la vía intraósea frente a la vía intravenosa en el contexto de la parada cardiorrespiratoria extrahospitalaria en población adulta es levemente desfavorable. Es muy recomendable la elaboración de estudios de mayor calidad que tengan en cuenta datos relacionados con las vías de acceso y las características de los pacientes.
