Abstract

fetched live from OpenAlex

Psychological stress injuries (PSIs) among Canadian paramedics and police continue to rise despite expanded research, funding, and mental health programming. This study explores this paradox through an institutional policy lens, drawing on qualitative data from semi-structured interviews with frontline and leadership personnel in public safety organizations. Findings reveal that entrenched institutional norms such as quasi-military organizational cultures, stigma surrounding mental health, and resistance to change undermine the effectiveness of PSI interventions. Rather than replacing outdated practices, new policies and resources are frequently layered onto existing structures, leading to fragmented and insufficient responses. Interviewees highlighted systemic issues such as long wait times, inadequate return-to-work support, and inter-organizational misalignment. These challenges persist not due to a lack of awareness or evidence, but because institutional dynamics constrain transformative change. The study contributes to the limited body of public policy research on PSIs by highlighting the role of organizational and cultural structures in policy implementation failure. It concludes with policy recommendations focused on institutional reform, enhanced coordination, areas for future research, and the integration of practitioner perspectives into mental health policy design. Key words: psychological stress injuries, posttraumatic stress injuries, paramedics, police, public safety personnel, institutional theory, qualitative policy analysis, organizational culture.

Not calibrated prevalence, not ground truth. Human validation pending. The Gemma side is a direct model label for every work in the frame, read from the title-only record. The Codex side is a classifier learned from the 10,348 direct Codex labels and calibrated to design-weighted sample rates; fields without enough sample support carry no Codex call. Candidate is the union of the two sides; consensus is their intersection. These outputs are machine_predicted_unvalidated and are not human labels.

Machine scores (provisional)

The two teacher heads of the student model, read on this work. A score orders the frame for review; it never asserts a category, and the validation status ships verbatim with every row.

Baseline scores from an immature model (maturity gate not passed, 7 training rounds). Scores rank; they never assert a category.
