Avaliação de perfil de sensibilidade de Klebsiella pneumoniae resistente aos carbapenêmicos em amostras clínicas de origem hospitalar no Distrito Federal

Abstract
Introdução: Klebsiella pneumoniae é uma das principais causas de surtos de infecção hospitalar, associada à redução de opções terapêuticas, ao aumento do tempo de internação, à necessidade de isolamento e à maior mortalidade. Objetivo: Avaliar o perfil de sensibilidade dos isolados resistentes aos carbapenêmicos em amostras clínicas de origem hospitalar no Distrito Federal. Método: Trata-se de um estudo retrospectivo. Foram analisados 989 isolados de K. pneumoniae identificados por MALDI-TOF, com resistência a carbapenêmicos determinada pelo sistema Vitek® 2 XL. Os antimicrobianos avaliados foram meropenem, ceftazidima/avibactam, ceftolozana/tazobactam, amicacina e polimixina B. Para detecção de resistência, foram utilizados os testes mCIM (método modificado de inativação de carbapenêmico) e eCIM (método com ácido etilenodiaminotetracético) e Carbapenembac®. Resultados: Das 989 cepas, 232 foram de hemoculturas, 396 de uroculturas, 185 do trato respiratório e 176 de secreções. Todas apresentaram produção de carbapenemase: 54,6% do tipo serina e 45,4% de metalo-β-lactamase (MBL). A sensibilidade foi de 60,39% para amicacina, 50% para ceftazidima/avibactam, 61% para polimixina B e apenas 3,41% para ceftolozana/tazobactam. Em 45 cepas MBL submetidas à genotipagem em laboratório de referência, 32 apresentaram o gene blaNDM e 13 co-produziram blaKPC e blaNDM. O estudo evidenciou a alta frequência de cepas produtoras de MBL, dentre as quais, os isolados que carreiam mais de um gene relacionado a esse mecanismo de resistência, refletindo a limitação terapêutica frente a esses isolados. Conclusão: Os resultados reforçam a importância de ações multidisciplinares para o controle da multirresistência e o uso racional de novos antimicrobianos.
